You will be a resource for high-risk pregnancy members, delegated plan Case Managers, Personal Care Coordinators (PCCs) and community partners to address medical, behavior and psychosocial concerns. <li>Utilizes all other available data to formulate care plans in the absence of an HRA.</li>
<li>Ensures and facilitates communication between PCCs and delegated plan PCCs to achieve favorable member health outcomes.</li>
<li>Responds appropriately to case management triggers generated as part of the HRA or ICT process.</li>
<li>Anticipates needs for those at risk of transitioning of care and proactively support reducing the risk of re-entering a higher level of care.</li>
<li>Acts as an integral part of the team, providing verbal and written support of member needs.</li>
<li>Collaborates and communicates with the member, family, significant other(s), physicians, health network (HN) designee and other health care providers to support and accomplish goals identified on the individualized care plan in all care settings.</li>

<li>Develops and maintains a network of current community resources and services where members can be referred for assistance.</li>

The Chief of Endocrinology will provide strategic leadership and direction for all aspects of the&nbsp;Division. A master’s degree from an accredited institution is required, with a doctoral degree preferred. Reporting directly to the Chancellor, the Vice Chancellor for Student Affairs is a key leadership position responsible for providing strategic direction and oversight for all aspects of student affairs at the university. This role focuses on enhancing the overall student experience, fostering a supportive and inclusive campus community, and promoting student success and well-being. The Vice Chancellor for Student Affairs works collaboratively with various stakeholders, including students, faculty, staff, and external partners, to develop and implement programs, services, and policies that meet the diverse needs of the student population.
The VCSA provides leadership for a variety of student support departments. The Student Affairs division is responsible for the traditional auxiliary units of a college campus, including University Housing, Dining Services, University Center (containing the bookstore and a textbook rental program), and Campus Recreation. Areas of responsibility for the student development support departments include: Dean of Students Office; Health Service; Counseling Services; Kimmel Belonging and Engagement Hub; an Early Childhood Center (an accredited daycare facility for children ages 2-5); and Career Development Center.

Position Summary
The Clinical Nurse Manager is responsible for the clinical management of an assigned caseload of clients. The Clinical Nurse Manager will lead and supervise all field staff toward specific outcomes on assigned cases under their supervision. They will ensure compliance with all clinical standards, policies, and procedures, including, but not limited to performing case opens and assessments, medication reconciliation, creation of clinical documentation, supply inventory, and medical status updates for patient population. This role also maintains compliance with the accrediting agencies, state, and funding source requirements.

Duties and Responsibilities
