Job Summary: 
The Medical Assistant will provide direct medical support to healthcare providers in an ambulatory care clinic setting, under the supervision of a physician or physician extender, as well as a Clinical Supervisor. Essential Duties and Responsibilities:

<li>Review the provider’s schedule and compile a list of patients to be reviewed during the huddle.</li>
<li>Review the EMR for patients using the appropriate template for preparation for the team huddle.</li>
<li>Check each chart for applicable lab test results and/or diagnostic test results and notify the provider of any problems or concerns. Track and obtain results if they are not in the system. </li>
<li>Identify patients with recent ER visits, hospitalization, or specialist visits; check for/obtain relevant information to support continuity of care.</li>
<li>Identify needs for patients with chronic conditions and facilitate care for patients, such as contacting a care coordinator, advocating for other Hamilton services, and soliciting providers to address needs.</li>
<li>Capture accurate medical history, current office visit information, and accurate vital signs, including heart rate, blood pressure, and temperature. </li>
<li>Document the chief complaint and prepare the patient for examination relevant to the chief complaint. Enter the chief complaint into the EMR. </li>
<li>Under the direction of the Provider may assist with medical examinations, to include but not limited to perform routine or specialized procedures including but not limited to venipuncture, wound dressings, ear irrigations, setting up sterile fields, and any procedure that is ordered by Provider while adhering to standards of within the scope of licensure and quality patient care. </li>
<li>Maintain patient and department flow-must be able to assist with nurse schedule as assigned, be flexible with daily schedule changes, and assist other team members when needed. </li>
<li>Clean rooms daily and after each patient. Follow protocols for proper cleaning procedures of all and any reusable medical equipment. </li>
<li>Maintains adequate stock of medical supplies within each exam room, adhering to proper storage protocols and ensuring compliance with dates, safety, and quality measures outlined by the Department and Nursing leadership. </li>

Minimum Education/Certifications: 
High school diploma or equivalent, along with completion of an accredited medical assistant program, is required. CPR certification is necessary and must be obtained within six months of job placement.
Minimum Work Experience:

POSITION SUMMARY

As a Hospice NP, here’s a big-picture view of what you’ll do:
<ul>
<li>Provides oversight of physician/Nurse Practitioner/Nurse Practitioner services to hospice patients by complementing attending physician/Nurse Practitioner care and ensuring continuity of hospice medical services including appropriate management of patient pain and symptoms related to the terminal condition.</li>
<li>Documents an individual patient’s progression in end-stage disease in the patient/family record as appropriate.</li>
<li>Ensures qualified physician/Nurse Practitioner participation in the initial development and ongoing review of the&nbsp;IDG Plan of care, and that the plan of care is developed prior to the delivery of hospice services.</li>
<li>Actively participates in&nbsp;IDG meetings, including the update of each patient’s comprehensive assessment and plan of care as often as a patient’s condition requires and in accordance with applicable law and regulation.</li>
<li>When needed, participates in:
<ul>
<li>The implementation and review of clinical protocols;</li>
<li>Patient continuing care decisions;</li>
<li>The oversight of the provision, adequacy, appropriateness and coordination of short term inpatient care, emergency services and respite care;</li>
<li>The quality assessment and performance improvement program (QAPI);</li>
<li>The resolution of patient/family or hospice program clinical and ethical disputes or concerns; and</li>
<li>The continuing education of the&nbsp;IDG, volunteers, and affiliated physician/Nurse Practitioners and employees of associated inpatient facilities.</li>
</ul>
</li>
<li>Acts as a patient’s physician/Nurse Practitioner to meet general medical needs, including pain and/or symptom management or other medical intervention should a patient’s attending physician/Nurse Practitioner not be available or chooses to relinquish responsibility for a patient’s care.</li>
<li>Acts as the liaison to community physician/Nurse Practitioners.</li>
<li>Completes other assignments as requested and assigned.</li>
<li>May have access to personal health information (“PHI”) necessary to fulfill the above duties and responsibilities. Access to use and ability to disclose PHI is further defined by each organization/department.</li>
</ul>
A few must-haves for Hospice NP:
<ul>
<li>An advanced degree in nursing– either an&nbsp;MSN (Master of Science in Nursing) or DNP (Doctor of Nursing Practice).</li>
<li>Current license in good standing.</li>
<li>DEA License Required&nbsp;</li>
<li>Hospice/palliative experience, preferred.</li>
<li>CPR certification</li>
<li>Valid driver’s license, auto insurance and transportation</li>
<li>Compassionate and helping nature, good communicator and ability to lift up to 50 lbs.</li>
<li>Ability to work as a member of the Interdisciplinary Group.</li>
<li>Knowledge of and commitment to hospice philosophy of care and palliative intervention.</li>
<li>Ability to effectively communicate with clinical and non-clinical staff, patients and family/caregivers as well as the medical community.</li>
<li>Meets applicable health requirements to provide patient care.</li>
<li>Meets the requirements of applicable federal, state and organizational background checks.</li>
