SUMMARY: 
As a Licensed Practical Nurse, you will provide professional patient care to long-term and post-acute care residents. You will direct and monitor the care provided by CNAs and consistently ensure proper documentation and recording is taking place. Your ability to lead is crucial in both the success of the CNAs and the care of the patients.
Attached to Clifton Springs Hospital, Clifton Springs Nursing Home offers a uniquely holistic approach to long-term care. Skilled nursing care is located adjacent to Spa Springs, where patients can access massage therapy, acupuncture and similar treatments. Clifton Springs Nursing Home is a 108-bed facility.
RESPONSIBILITIES:
· Responsible for timely, accurate medication administration of assigned team and concurrent required documentation.
· Performs treatments and documents progress of same as per physician orders and facility protocol.
· Assures concise, pertinent and complete resident care documentation using computerized medical record process. Assures the initiation of appropriate care pathway tools.
· Responsible for receiving/giving relevant reports, both written and verbal from/to appropriate staff as per departmental protocols.
· Assures proper monitoring of residents and relevant condition status are reported to charge nurse/supervisor and/or physicians in a timely manner.
· Completes pre-reviews of medication administration records as per facility protocols.
· Completes required MDS documentation and all related observations in a timely manner.
· Supports CNA team through assisting in direct ADL care of residents.
REQUIRED QUALIFICATIONS:
· Graduate from an accredited School of Practical Nursing.
· Current licensure in State of New York.

POSITION SUMMARY
The Registered Nurse (RN) assesses, plans, provides, and evaluates preventative, therapeutic and patient rehabilitative care from age adult through geriatric. The RN promotes and restores patients' health by completing the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to patients; providing safe medication and emergency and withdrawal management. The RN appropriately delegates tasks to nursing support staff. 
KEY RESPONSIBILITIES 
<ul>
<li>Dispenses medication according to the 5 R's (right patient, right medication, right dose, right route, right time).</li>
<li>Assures competency in the use of emergency and clinical equipment as evidenced by the proper use of Glucometer, Breathalyzer, Nebulizer, emergency response equipment and drugs.</li>
<li>Assures competency in the management of the following emergency conditions: Chest pain, SOB/Dyspnea, Seizures, Hypo/Hyperglycemia.</li>
<li>Assures knowledge of pain management as evidenced of ability to assess pain level utilizing pain scale, development of the treatment plan, explain comfort measures and reassesses per policy.</li>
<li>Contract for Safety and Aggressive Person Management Policy</li>
<li>Demonstrates ability to verbalize recommended script to negotiate a "contract for safety" with patients and also demonstrates understanding of Aggressive Person Management policy.</li>
<li>Performs accurate assessments upon the patient’s admission, during detoxification and during rehabilitation. Competent use of withdrawal management protocols.</li>
<li>Develops appropriate treatment plans. Completes required documentation per policy. Knows how and when to report cases of abuse (sexual, physical, elder) and child neglect.</li>
<li>Demonstrates competency in applying development and age-related knowledge in the care of patients in the following age groups: Adolescent (18-19 years), Young Adult (20-39 years), Middle Adult (40-64 years), older adults age (65-74 years) and Geriatric (75 and older).</li>
<li>Protects personally identifiable health information; adheres to release of information per policy and procedure (written, oral and electronic). Respects patients and all customer groups' privacy and confidentiality. Adheres to IT and business-related communications confidentiality policies.</li>
<li>Alert to opportunities to reduce risks of accidents or potential safety and security breaches. Routinely follows safety and security policies and procedures. </li>
<li>Reports all safety related incidents in a timely and complete manner; completes accident and incident reports correctly. </li>
<li>Participates in mandatory safety training sessions; participates in fire and disaster drills.</li>

DESIRED KNOWLEDGE/SKILLS/ABILITIES:

<li>Associates Degree in Nursing required; Bachelor’s Degree preferred </li>
<li>Active Registered Nurse license in Maryland or compact states required</li>
<li>Active BLS certification required</li>
<li>One-year experience in Acute Care Nursing preferred. Addiction or Psychiatric Nurse experience desirable.</li>
<li>Ability to read medical documents, such as doctor's orders, psychological evaluations, dietitian assessments and write routine reports. Communicate effectively with patients and staff.</li>
<li>Accurately measure medical dosages as prescribed by the Medical Staff. Basic knowledge of Microsoft Office to include Outlook, Word and Excel.</li>
<li>Common sense understanding to carry out detailed medical instructions. Be sensitive to the needs of chemically dependent patients. Ability to learn and adapt. Ability to direct others. Deal with problems involving a few concrete variables in standardized situations.</li>
