We have a new opening for a Registered Nurse - Case Manager at Joint Base Elmendorf-Richardson in Anchorage, AK.
Core Duties: The duties include, but are not limited to the following;
• Participate in all phases of the Case Management Program (CMP) and ensure that the CMP meets established case management (CM) standards of care. • Provide nursing expertise about the CM process, including assessment, planning, implementation, coordination, and monitoring. Identify opportunities for CM and identify and integrate local CM processes. • Develop and implement local strategies using inpatient, outpatient, onsite and telephonic CM • Develop and implement tools to support case management, such as those used for patient identification and patient assessment, clinical practice guidelines, algorithms, CM software, and databases for community resources. • Integrate CM and utilization management (UM) and integrating nursing case management with social work case management. • Maintain liaison with appropriate community agencies and organizations. • Accurately collect and document patient care data. • Develop treatment plans including preventive, therapeutic, rehabilitative, psychosocial, and clinical interventions to ensure continuity of care toward the goal of optimal wellness. • Establish mechanisms to ensure proper implementation of patient treatment plan and follow-up post discharge in ambulatory and community health care settings. • Provide appropriate health care instruction to patient and/or caregivers based on identified learning needs. • Coordinate patient care in collaboration with a wide array of healthcare professionals. • Facilitate the achievement of optimal outcomes in relation to clinical care, quality, and cost effectiveness. • Ensure compliance with standards of care and practice in accordance with all established policies, procedures, and guidelines used in the facility. • Perform physical exam and health histories. • Recognize adverse signs and symptoms and quickly react in emergency situations. • Communicate and collaborate with a diverse group of people for the purpose of informing the healthcare team of plans/actions, for teaching/education to benefit the patient/family and organization. • Make referral appointments and arrange specialty care as appropriate. • Perform nursing services identified in the TO Conduct research in support of improved practice and patient outcomes.

Minimum Qualifications:
Degree: Possess a Bachelors degree in Nursing from a college or university accredited by Accreditation Commission for Education in Nursing (ACEN) or the Commission on Collegiate Nursing Education (CCNE).
Experience: A minimum of one year of experience as a Case Manager. Experience with military Active Duty service members and knowledge of Department of Defense (DoD), Federal, State, non-profit Healthcare systems, and organizations, to include DoD Tri-Service Military Healthcare Systems, TRICARE purchased care system, and the Department of Veterans Affairs, along with basic knowledge of the Recovery Care Coordinator program.

This role focuses on supporting beneficiaries with mental health or substance use crises, implementing interventions outlined in the Person-Centered Plan (PCP), and facilitating connections to community resources. The QP ensures compliance with NC Medicaid Clinical Coverage Policy 8A and contributes to the MCM team’s first-responder model.
Essential Duties and Responsibilities: 
<ul>
<li>Respond in-person to crisis calls in community settings, providing triage, de-escalation, and supportive interventions to stabilize beneficiaries (page 35).</li>
<li>Assist in conducting crisis assessments using NC-SNAP or ASAM Criteria, under the direction of the LP, to determine service eligibility (page 36).</li>
<li>Implement interventions specified in the beneficiary’s PCP, such as coping skills training, safety planning, or linkage to community supports.</li>
<li>Collaborate with the LP to develop and update PCPs, ensuring interventions address crisis-related goals (Subsection 5.7.1, page 7).</li>
<li>Supervise Associate Professionals (APs) and Paraprofessionals under an individualized supervision plan, reviewed annually (Subsection 5.6, page 6).</li>
<li>Document interventions in service notes, including date, purpose, intervention details, duration, and effectiveness, per Subsection 5.8.2 (page 8).</li>
<li>Coordinate with emergency services, LME/MCOs, or other providers to facilitate follow-up care and prevent hospitalization.</li>
<li>Participate in 24/7 first-responder coverage, ensuring prompt response to crisis calls (page 35).</li>
<li>Support telehealth components (e.g., follow-up consultations) as directed by the LP, adhering to Policy 1-H requirements (Attachment A, page 30).</li>
