The WelbeHealth Clinical Nurse Manager is accountable for the delivery of high-quality ambulatory clinic care including maintaining efficient clinic operations. Under the direction of the Clinical Services Director (CSD), the WelbeHealth Clinical Nurse Manager organizes and directs the day-to-day operations within the clinic, oversees the clinic staff, and carefully coordinates clinic workflows to ensure effective WelbeHealth clinical services are provided.
The Clinical Nurse Manager is well-versed in people development, management, and RN level nursing care. The Clinical RN Manager maintains accountabilities for all functions of the WelbeHealth clinic and clinic team, and ensures policies and workflows are clearly understood and followed by all clinic and RN Case Manager team members.
Essential Job Duties:

<li>Hire, retain, coach, develop, supervise, and ensure the RN Case Managers, Clinic RN, Medical Assistants, and other clinic team members are engaged and healthy</li>
<li>Analyze data with attention to trends, strengths, weaknesses, and opportunities, performing root cause analysis, evaluating care delivery processes, and based on findings, making recommendations to the CSD on changes to strategies, workflows, and policies to continuously improve clinic operations</li>
<li>Ensure nursing and clinic staff deliver and correctly document interventions as agreed upon in participants' care plans, including but not limited to promptly and accurately responding to physician orders, completing assessments, , and actively participating in care planning for all assigned participants</li>
<li>Evaluate the quality and effectiveness of coordination and services in conjunction with medical practice leadership, providers, case managers, and other relevant IDT disciplines</li>
<li>In collaboration with CSD and Quality Improvement Director, serve as the audit lead for clinical and nursing elements during CMS/DHCS audit review</li>
<li>Develop and monitor corrective action plans to ensure ongoing compliance</li>

Job Requirements:

<li>Bachelor's degree in nursing or other healthcare related field, preferred; professional experience may be substituted</li>
<li>Unencumbered registered nurse (RN) state license required</li>
<li>Minimum of two (2) years of experience overseeing an ambulatory clinic operation including RN, LVN, and MA staff; equivalent nurse leader experience in a high volume, outpatient setting may be substituted</li>
<li>Minimum of two (2) years of experience in a clinical setting with a frail or elderly population</li>

TITLE: Nurse Manager &amp; Discharge Planning (Nurse Practioner)
REPORTS TO: Chief Executive Officer &amp; Director of Nursing
ROLE DESCRIPTION:
The Nursing Manager is integral to the functioning and oversight of the clinical team and is also responsible for utilization review, quality assurance, and discharge planning. The incumbent should have solid clinical knowledge and best practices to help the agency define and refine a value-based culture in the clinical department, ensuring that team members comply with regulatory and ethical standards as they support the delivery of patient care.
The candidate must be proactive, passionate about creating and maximizing interdisciplinary relationships to support integrative care for clients, and capable of participating in and leading transformational activities in the nursing department. The candidate should be proficient in connecting with, coaching, and encouraging their team, positioning them for efficiency, productivity, and success.
RESPONSIBILITIES
General functions
Responsibilities specific to Discharge Planning include assessment, identification of specific needs, and social service intervention or referral. Awareness of services available to patients and their families is an important part of this assessment. Determining patient needs on the basis of diagnosis; prognosis and social support system/person information is included in the initial assessment process. Upon individual recognition and staff referral, the discharge planner will contact the appropriate agency to meet the patient's social and emotional needs.
Partners and collaborates with the DON to ensure that policies and procedures of the organization are up to date, in line with adjustments in regulations from NYSDOMH, Federal, and other regulatory bodies, and are adequately communicated and reinforced with the nurses as a part of a more significant communication and training strategy. Collaborates with the DON to maintain policies and procedures for the effective implementation and tracking of monthly statistics for the nursing department. Works closely with the QI team to support maintaining program quality standards, benchmarks, and performance measures regarding nurse activities and outputs. Resolve conflicts and matters that could detriment the client and agency because of non-compliance of the nurse with established policies and procedures. Lead the investigation and resolution of client incidents and complaints. Collects data on common errors and omissions and present them at nursing meetings. Assists actively with the remediation of any issues with the data management system; can learn the documentation system and seek assistance from customer service to resolve any problems with the technical aspects of nursing documentation and input. Actively participates as a vital member of the agency Audit Team to ensure the correct submission of documents required during all in-person and remote and lead with remedying gaps and errors identified during the audit. Execute visits for clients. Provide training and coaching to the nurses supervised. Continuous Quality Improvement
<ul>
<li>Provides feedback to the CEO and director of Nursing on trends occurring in clinical documentation, including difficulties with completing documentation, timeliness of clinical documentation and nursing visits, and any issues relating to the compliance of nursing documentation and service delivery.</li>
<li>Participates in Continuous Quality Improvement activities under the supervision of the Director of Nursing and ensures that recommended improvements and post-audit corrective action plans are implemented.</li>
<li>Establishes and ensures implementation and completion of any/all corrective action plans for individual client cases as necessary.</li>
<li>Provide oversight over charts and preside over client case conferences as required.</li>
<li>Conducts internal chart audits as a monitoring tool to ensure the appropriate documentation by all staff per Agency procedures and all external mandates.</li>
<li>Participate in the Nursing Quality Oversight Committee to assist in identifying issues, corrective actions, and implementing recommendations to improve the quality of the Nursing department function.</li>
<li>Other duties as assigned by the Director of Nursing and Chief Executive Officer</li>

Salary: $100,000.00 - $120,000.00 per year
Expected hours: No more than 80 per week
