Roles and Responsibilities
• Conducts assessments, including comprehensive annual wellness exams on patients both in the patients' home and in the virtual environment

• Documents items such as: appropriate chief complaint, all applicable diagnoses, past medical, family, and social history, review of systems, examinations, medications, allergies, assessment, and plan

Job Summary:
Provides individualized and specialized nursing care to patients in the ambulatory surgery center (ASC) in accordance with physician’s orders, recognized nursing standards, legal aspects of current nurse practice within the state, federal, and the ASC’s policies and procedures. Functions cooperatively with physicians and staff exhibiting teamwork characteristics. Demonstrate initiative, insight and judgment in decision-making process.

Primary Job Duties:

<li>Evaluation and complete assessment of patient needs in providing supportive care prior to, during, and after surgery and maintains accurate and complete records</li>
<li>Competent in pre op, circulator, post op, and call nurse duties</li>
<li>Communicates pertinent information to co-workers</li>
<li>Provides a safe environment for the patient to prevent potential injury</li>
<li>Investigates and intervenes in problem situations, reporting appropriate information to supervisor and documents the same</li>
<li>Maintains equipment and supplies to include:

<li>Cleaning and disinfection of equipment </li>
<li>Operates and troubleshoots specialized equipment and patient monitoring equipment</li>
<li>Anticipates and assists with maintaining adequate inventory of supplies and medication</li>

Performance Standards/Responsibilities:

<li>Use of surgery schedule to guide patient's perioperative experience including timeouts and patient plan of care</li>
<li>Pre-operative: Pre and day of surgery physical, cognitive, and social assessment of the patient with appropriate actions to render safe and correct surgery, including anesthesia collaborative plan</li>
<li>Circulator Nurse: As the role in charge of the OR room, coordinates all functions of the team to render safe and correct patient care, and safe environment controls for optimal outcomes </li>
<li>Post-operative: Oversees the admission to post surgery area and continues to monitor the patient for recovery from anesthetic</li>
<li>Performance demonstrates accuracy in the nursing process and documentation of such care</li>

Job Duties

<li>Evaluate and manage chronic pain conditions as part of comprehensive care plans, integrating pharmacologic and non-pharmacologic therapies.</li>
<li>Collaborate with pain management specialists and complementary providers (chiropractors, acupuncturists, massage therapists) to develop individualized multimodal pain treatment strategies.</li>
<li>Perform interventional pain procedures as trained and credentialed, including joint injections, trigger point injections, and other minimally invasive therapies.</li>
<li>Educate patients on safe pain management practices including opioid stewardship, alternative therapies, and self-management techniques.</li>
<li>Monitor treatment efficacy and adjust care plans to optimize function, quality of life, and long-term health outcomes.</li>
<li>Deliver care using a holistic, interdisciplinary approach in collaboration with medical doctors, chiropractors, acupuncturists, behavioral health providers, dietitians, and other health partners within our patented care model.</li>
<li>Participate in a multidisciplinary team to develop, execute, monitor, and adjust comprehensive and coordinated care plans that include shared goals with measurable outcomes.</li>
<li>Assess and document patient health through interviews, examinations, observations, and review of medical histories.</li>
<li>Diagnose health conditions, order and interpret lab work and diagnostic studies following established protocols.</li>

Qualifications

<li>Current DEA registration (Drug Enforcement Administration) or eligibility/ability to obtain DEA registration required.</li>

<li>1+ years of relevant work experience as Physician Assistant/ Nurse Practitioner preferred but not required.</li>

About the Role:
The Care Coordinator for the IDD Pilot Program plays a pivotal role in managing and facilitating comprehensive care plans for individuals with intellectual and developmental disabilities. This position ensures that participants receive coordinated, person-centered services that promote their health, well-being, and independence. The Care Coordinator acts as a liaison between healthcare providers, community resources, families, and the individuals themselves to streamline access to necessary supports and services. By monitoring progress and adjusting care plans as needed, the role contributes to improved health outcomes and quality of life for program members. Ultimately, the Care Coordinator’s core functions include assessing individual needs, developing a person-centered support plan, coordinating services and care, and serving as the enrollee’s advocate.
Minimum Qualifications:
