Responsibilities
Basic Function - This position is involved with facility maintenance duties and customer interface. Previous experience is preferred, but not required in all positions. Individual must be able to perform with minimum supervision. Other duties may include: equipment service and general housekeeping tasks.

<li>Perform all maintenance and cleaning tasks noted in the checklists and as directed by the facility manager/supervisor to ensure that all dirt, dust, litter and debris is removed from all areas of the facility using brooms, mops, brushes and other hand tools.</li>
<li>Walk garage daily to note and correct any maintenance problems. </li>
<li>Sweep and pick up around the entrances and exits to the garage and stairwells as well as clean surface area rails, etc. on all levels, the booth, the office and rest rooms. </li>
<li>Empty all trashcans in the garage, the booth and the office. </li>
<li>Paint any and all areas of the facility as directed by the facility manager/supervisor to ensure that all painted surfaces such as poles, signs, doors, walls, curbs, islands, gates, etc. are properly maintained and look neat and clean.</li>
<li>Replace burned out bulbs. </li>
<li>Apply oil absorbent to oil spills. </li>
<li>Submit requests for repairs or maintenance. </li>
<li>Check conditions of safety items, such as gate arms to ensure clearance bars are secure. </li>
<li>Check ticket spitters and gates daily for proper operations, ticket supply, accuracy of time on clock and general appearance. </li>
<li>Provide ticket and record gate counters to the Manager. </li>
<li>Keep an inventory of janitorial supplies. </li>
<li>Maintain a garage/lot sweep sheet. </li>
<li>Operate automatic sweepers, scrubbers, outdoor equipment such as weed eater, lawnmowers or other electrical tools to repair or construct items, as applicable. </li>
<li>Shovel, plow or blow snow from the entrance or exit ramps and in areas designated by the Facility Manager using shovels, snow blowers and other light duty equipment.</li>
<li>Report any unknown, observed or suspected violations of company policy, safety hazards or any unusual occurrence to the Facility Manager in a timely manner.</li>
<li>Practice the Three Keys to Customer Satisfaction including the Five Customer Service Behaviors to ensure successful customer interactions.</li>
<li>Assist customers within the facility or as directed by the Facility Manager/Supervisor.</li>

The program achieves positive results by combining comprehensive case management with proven supervision technology. In addition, BI maintains an extensive list of community-based partners to provide critical assistance for participants' basic life needs. With consistent positive outcomes, ISAP continues to grow and evolve, serving an ever-increasing number of participants.

 
Responsibilities
Summary:
The Government Site Specialist provides case management of Intensive Supervision Appearance Program (ISAP) participants from entry to release by administering the program’s policies and procedures as defined by the Department of Homeland Security (DHS) contract. This position is operating independently at a government site without a Program Manager on site.

Primary Duties and Responsibilities:

<li>The Government Site Specialist works independently to manage all aspects of case management within the program requirements.</li>
<li>Develops, implements, coordinates, reviews and updates case management and service plans for ISAP participants in conformance with state and federal regulations and program requirements.</li>
<li>The Government Site Specialist meets with participants regularly as required by contractual obligations.</li>
<li>Develops and maintains accurate and complete case records for all ISAP participants from entry to release from ISAP. Proactively manages cases using all appropriate tools and techniques. Maintains confidentiality of all records.</li>
<li>The Government Site Specialist manages program participant violations, including no-shows and repeat violators. Prepares documentation to acquire Department of Homeland (DHS) direction and action if disciplinary measures are necessary.</li>
<li>Administers Security Plan for a specific office, including daily operations, security contingencies, emergencies and mitigation plans, security of Company and DHS employees, clients and the general public. Performs daily security check of the office area. Ensures that all participant records are kept in locked files to maintain confidentiality. Prepares written reports for DHS as necessary.</li>
<li>The Government Site Specialist reviews and approves program participants’ service plans to ensure compliance with state requirements and contractual obligations.</li>
<li>Conducts intake interview and orientation session with each ISAP participant within contractual timeframes and requirements.</li>
<li>The Government Site Specialist installs electronic monitoring equipment on ISAP participants as necessary and monitors compliance with electronic monitoring program.</li>
<li>Assists ISAP participants with acquiring travel documents from the countries of citizenship. Maintains a log of travel document information for each participant.</li>
<li>Evaluates English proficiency of ISAP participants to determine resources necessary to promote clear communication between ISAP staff and the participants. </li>

Qualifications
Minimum Requirements:

<li>Bachelor’s Degree required. Degree in Sociology, Psychology, Social Work, Criminal Justice or related field preferred.</li>

Responsibilities
About the Role

During a visit that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment and other risks. This provides unmatched insight into a member’s overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment (CHA) helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit. Our providers:

<li>Conduct adult / geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols</li>
<li>Administer and order point of care and lab-based testing as needed</li>
<li>Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate</li>
<li>Collaborate with Primary Care Physician (PCP) on patient education and follow up as necessary</li>
