Position Summary: Provide educational instruction through a variety of accepted teaching techniques while employing behavior management strategies that will ensure the effective and efficient implementation of assigned individual student’s IEPs. 
 
Primary responsibilities:
<ul>
<li>In cooperation with other members of the school team, assess educational needs and the progress of assigned students.</li>
<li>Provide instruction to assigned students in classes of expertise as assigned.</li>
<li>Prepare lesson plans suitable to the environment, age group and class mix on the assigned subject area(s). </li>
<li>Assist students individually where necessary; serve as a champion and advocate for each student taking a vested interest in the success of each student’s learning, and social and emotional growth. </li>
<li>Remain apprised of the latest and emerging, most widely accepted teaching techniques, in an effort meet the needs of emotionally and behaviorally challenged students within a residential setting. </li>
<li>Provide assessments of progress to academic goals. </li>
<li>Continually look for new approaches to instruction, class content and be responsive to students’ needs.</li>
<li>Collaborate with internal staff and communicate with students, coworkers, and leaders in a respectful and professional manner; lead by example remaining supportive of the Nexus mission, methods and values. </li>
<li>Provide crisis intervention within the classroom setting and apply HWC de-escalation techniques appropriate to the situation at hand; ask for assistance when warranted through appropriate procedures and keep other academic and clinical treatment staff appraised of inappropriate behaviors.</li>
<li>Create a classroom climate that is positive and supportive for both learning and treatment. </li>
<li>Establish and maintain appropriate relationships with students and emphasize student responsibility in the educational program. </li>
<li>Recognize student achievement and provide appropriate reinforcement, acknowledgement and recognition, while redirecting student behavior opportunities and maintaining appropriate boundaries. </li>
<li>Communicate students’ academic and behavioral successes and failures through both written and verbal communication to both fellow educational staff and treatment team staff. </li>
<li>Incorporate cultural diversity into all aspects of educational programming and instruction; attend trainings and other educational opportunities to become better informed and respectful of other cultures. </li>
<li>Ensure confidentiality regarding students is in compliance with all licensing agencies, company policy, COA and HIPAA guidelines.</li>
<li>Develop or assist in developing IEPs, test ability levels and coordinate meetings with other members of the IEP team where appropriate.</li>
<li>Attend faculty/staff meetings, IEP staffings, and participate in additional meetings and training sessions as required; understand and support the Nexus treatment philosophy and modalities. </li>

Requirements:
Required Education and Licensure:

<li>Minimum of a Bachelor’s degree in an educational discipline is required. </li>
<li>Active teacher’s licensure/certification required. </li>
<li>Valid driver’s license required. Must meet state regulating agency and Home Office driving requirements.</li>
</ul>
Nice to Have:

<li>2 years previous teaching experience preferred. </li>
<li>Active teacher’s licensure/certification specialization in the areas of EBD, LD, SED and TMH. </li>

Position Summary: Provide overall management of youth’s treatment through active engagement, clinical assessment, treatment, and discharge planning. Therapist will provide clinical insight and work from a trauma informed lens to enhance the milieu and programming. Engage with families through family therapy and treatment planning. Collaborate with the community and providers related to the youth’s progress and discharge planning. 
 
Trauma Informed Care
<ul>
<li>Helps establish and maintain a positive treatment culture &amp; environment</li>
<li>Works with youth &amp; family to develop a Nexus Individual and Family Guidance Plan</li>
<li>Active involvement with Family Collaborative Intake meetings</li>
<li>Completes Child and Adolescent Needs and Strengths (CANS) functional assessment in accordance to TCOM (collaborative) principals</li>
<li>Through the collaborative effort with family, youth, and providers, develop and monitor the individual treatment plans</li>
<li>Provide individualized, youth-focused, strength and evidenced-based family and individual therapy models to youth and families. Examples: Trauma Focused Cognitive Behavior Therapy and Eye Movement Desensitization (TF-CBT) and Reprocessing (EMDR)</li>
</ul>
Milieu Coordination 
<ul>
<li>Develop and facilitate milieu and treatment groups that are trauma informed, educational, and treatment focused</li>
<li>Participate in all youth related treatment team meetings, using family centered language and youth driven principals</li>
<li>Serve as a clinical treatment team leader to YCP's. This includes but is not limited to modeling and coaching staff on rapport building with clients, trauma-informed practices, and de-escalation techniques on the treatment milieu as needed</li>
<li>Be available on-call for consultation/treatment direction and/or decisions when appropriate</li>
<li>Assess readiness for discharge in conjunction with the treatment team</li>
</ul>
Community Care
<ul>
<li>Coordinate/communicate aftercare needs of the client with family and all necessary social service and community providers to ensure a smooth transition</li>
<li>Communicate with the treatment team to include all desired and necessary family members, educational, legal, community, and social service providers (as well as all-natural supports that the youth and family identify) to gather all necessary information to inform and support the treatment process</li>
<li>Ensures that all required client reports, records, and all documentation are produced accurately and maintained in a consistent, timely, and professional manner according to policy and regulated statutes</li>
<li>Educate and facilitate trainings for staff on trauma informed practices, clinical diagnosis treatment processes, and other clinically relevant training material</li>
<li>Participate in team meetings, program clinical team meetings, therapy clinical supervision sessions, and in-service as required</li>
<li>Ensure confidentiality regarding mental health, medical issues, background treatment and comply with all HIPAA related requirements</li>
<li>Remains current on all company policies, assigned trainings and/or certification requirements</li>
</ul>

The Nurse Practitioner or Physician Assistant works both independently and in collaboration with surgical consultants, fellows/residents, nursing and other staff to provide high quality outpatient care in a busy Breast and Melanoma surgical practice.&nbsp; The APP is responsible for performing independent preoperative assessment, coordination of patient care, and patient management for breast and melanoma surgical patients in both the preoperative and postoperative outpatient setting.&nbsp;Time is spent both in independent clinic and in clinic with the surgeons in a collaborative setting. Provision of care includes, but is not limited to: comprehensive physical assessment, ordering and interpreting diagnostic tests, performing diagnostic and therapeutic procedures, and educating and counseling patients and their families.&nbsp;&nbsp;

The APP will work alongside another Breast and Melanoma surgery APP and multiple surgical APPs providing care in the outpatient clinic.&nbsp; The APP will also work very closely with our colleagues in Plastic Surgery, Medical Oncology, and Radiation Oncology. This position is 100% outpatient clinic practice with no call, holidays or weekends.
 Qualifications 
Masters prepared graduate of an accredited school of nursing and nurse practitioner program. Current Minnesota RN license and certification, or eligible to be certified, as an ANP, ACNP, or FNP.&nbsp; Or, graduate of an accredited Physician Assistant program with current PA certification by the NCCPA and registration as a PA by the Minnesota State Board of Medical Practice. The Surgical Technologist provides safe and efficient care to the surgical patient under the supervision of the registered nurse (RN) and in collaboration with the surgeon. The Surgical Technologist functions primarily in the scrub role and is responsible for selecting, preparing, and maintaining instrumentation, equipment, and supplies. The Surgical Technologist possesses knowledge in the application of sterile and aseptic technique and is able to adapt to unpredictable situations within the perioperative setting. Nurse responsibilities are to provide direct care to patients and families through their cancer journey. RNs manage patient symptoms with support of provider, provide treatment education to patients and families, and help patients with oncology related questions.&nbsp; They support to the Oncology Care team and help guide decisions for patients, guide patients to the resources available, and see patients at frequent intervals through their care. Medical Oncology staff work in a Care Team model with the support of multiple disciplines to help support the patients. The Care Team RN helps organize the patients care in this model.
The registered nurse (RN) is accountable for the coordination of nursing care, including direct patient care, patient/family education and transitions of care. The RN supports professional nursing practice across practice settings and across the continuum of care to meet the needs of the patient and family. The RN will function within the Mayo Clinic Nursing Professional Practice Model, which includes accountability for planning, implementing, evaluating and communicating all phases of nursing care for assigned patients. The ANA Nursing: Scope and Standards of Practice provide a basis for the practice of the RN. The RN provides leadership through activities such as preceptor role, informal and formal leadership roles, and quality improvement efforts. The RN delegates patient care according to skill level, experience, patient acuity, fiscal accountability and availability of resources. The RN possesses excellent communication skills; is skillful in mentoring and teaching; and may participate on committees or projects.
