Housing Specialist
Job description
We are seeking Housing Specialists who will be responsible for permanent housing placement for all housing-ready individuals in the shelter. The Housing Specialist will have an assigned caseload and will be responsible for the successful placement of these individuals by DHS' contractual requirements. The Housing Specialist works with community organizations, government agencies, and property owners to secure living arrangements, and negotiate rents, deposits, and move-in terms on behalf of their clients. The position involves various tasks that increase housing options for people who are trying to transition to permanent housing.

Responsibilities consist of but are not limited to:

Responsibilities of the Physical Medicine &amp; Rehabilitation Physician:

<li>Examine patients that need to undergo physical therapy.</li>
<li>Diagnose pain as a result of an injury, disease, or a disabling condition.</li>
<li>Determine and implement appropriate treatment plans for patients.</li>
<li>Recommend and undertake various medical procedures, such as disc decompression, prolotherapy, and vertebroplasty to treat pain and improve physical functioning.</li>
<li>Lead a team of medical professionals, which may include physical therapists and occupational therapists, in order to optimize patient care.</li>
<li>Monitor ongoing progress and health of patients, and change treatment plans, as necessary.</li>
<li>Provide adaptive tools to improve the overall health and physical functioning of patients.</li>
<li>Counsel patients and their families and help them adapt to lifestyle changes.</li>
<li>Provide support and advice to patients receiving long-term care.</li>
<li>Conducting research on injuries, diseases, and disabling conditions in order to improve and advance treatments</li>

Key Responsibilities:

<li>Conduct individual and group psychotherapy sessions for patients in a subacute or skilled nursing facility.</li>
<li>Assess, diagnose, and treat a wide range of emotional and behavioral health conditions, including depression, anxiety, adjustment disorders, and cognitive decline.</li>
<li>Develop and implement evidence-based treatment plans tailored to the needs of each patient.</li>
<li>Collaborate with facility staff, interdisciplinary teams, and family members to support patient care.</li>
<li>Maintain accurate and timely clinical documentation in accordance with regulatory standards using our clinician-designed electronic medical/health system (EHR). </li>

Requirements:

<li>Active and unrestricted license to practice as a Licensed Clinical Social Worker in State applying for.</li>
<li>Effective oral and written communication in English</li>
<li>Basic proficiency with technology, including electronic health records (EHR).</li>
<li>Strong organizational and documentation skills, with attention to regulatory compliance.</li>
<li>Ability to provide in-person services at assigned facilities, up to 45 minutes.</li>
<li>Ability to walk, stand, and move between patient rooms and offices within the facility throughout the day.</li>
<li>Full Vaccination and Booster Status may be required in some facilities.</li>
</ul>

Preferred Qualifications:
<ul>
<li>Previous clinical experience in a subacute, long-term care, or geriatric healthcare setting.</li>
<li>History of treating Anxiety, Depression and Adjustment Disorders.
