1 – 3 years’ experience in senior living/skilled nursing activity &amp; event planning which may include moderate to large event planning responsibilities outside the senior living industry

Provides nutrition counseling, education, and therapy to participants; educates and counsels family and other caregivers regarding dietary issues; and conducts initial assessments and periodic reassessments.  Participates in the development of the plan of care, assists in the coordination of 24-hour care delivery, and regularly informs the Interdisciplinary Team (IDT) of the nutritional needs of each participant. ESSENTIAL DUTIES AND RESPONSIBILITIES &nbsp;

<li>Provide high quality nutritional care and services and serves as a member of the PACE interdisciplinary team (IDT) </li>
<li>Conduct an initial comprehensive nutritional assessment upon enrollment and as needed according to PACE procedures regulations. </li>
<li>Collaborate with the interdisciplinary team to develop a comprehensive care plan for each participant. Revise goals and approaches to participants’ care in coordination with interdisciplinary team. </li>
<li>Provide nutritional counseling to participants, care givers and family members to promote health and wellbeing of participants. </li>
<li>Facilitate nutrition-related care plan interventions, such as home delivered meals and supplements.  </li>
<li>Monitor weight, medical diagnosis, drug nutrient interactions, changes in chewing or swallowing, mood changes, hydration status, intake, skin breakdown and report changes to the interdisciplinary team with recommendations for changes. </li>
<li>Works collaboratively with the interdisciplinary team (IDT) to develop and implement the comprehensive plans of care for participants.  </li>
<li>Develop and maintain positive relationships and communication with co-workers, participants and their families/significant others, and members of the community.  </li>
<li>Participate in all interdisciplinary team meetings.</li>

Responsibilities

<li>PATIENT ASSESSMENT AND PLANNING:
<ul>
<li>Assess patient eligibility for specific service needs, developing individualized integrated action plans.</li>
</ul>
</li>
<li>RESOURCE REFERRAL AND MONITORING:
<ul>
<li>Refer and connect patients to resources, monitoring adherence and providing necessary follow-ups.</li>
<li>Assist patients in accessing medical, social, education, and employment resources.</li>
</ul>
</li>
<li>DOCUMENTATION AND REPORTING:
<ul>
<li>Complete detailed reports on needs assessment and case management activities.</li>
<li>Ensure timely and comprehensive input of all necessary case management data into the electronic patient health record.</li>
</ul>
</li>
<li>PATIENT SUPPORT AND COORDINATION:&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;
<ul>
<li>Provide crisis and de-escalation techniques.</li>
<li>Ensure patients have reliable transportation to BrightView and other care providers.</li>
</ul>
</li>
<li>PROFESSIONAL DEVELOPMENT AND COMPLIANCE:
<ul>
<li>Attend required meetings, including supervision, treatment team meetings, and quarterly in-service.</li>
<li>Adhere to regulatory and company requirements in documentation and evaluation.</li>
<li>Participate in TCM audit, meeting assessment and treatment plan completion expectations.</li>

<li>Excellent verbal and written communication skills</li>
<li>Highly empathetic and compassionate to effectively support the recovery journey of BrightView’s patients</li>

<li>Natural problem solver, looks for solutions to best meet patient needs with a sense of urgency</li>
<li>Team-player, able to work collaboratively in a multidisciplinary healthcare environment</li>
<li>Adaptable and agile within a dynamic work environment</li>
<li>Technologically capable, comfortable operating in multiple systems for communication and documentation purposes. Familiarity with MS Office software (Outlook, Teams, Word, etc) preferred</li>
<li>Embraces BrightView’s culture of compliance - operates with a high degree of integrity and compliance to work standards and regulatory requirements</li>
<li>Familiarity with HIPAA and 42-CFR Part 2 desired</li>
<li>Prior experience with harm reduction a plus</li>
