Essential Functions :

<li>Complete all intake paperwork, including initial services plans and vulnerable adult screenings.</li>
<li>Complete comprehensive assessments and summaries, develop and update individual treatment plans, complete weekly treatment plan reviews, provide treatment coordination, and complete insurance forms.</li>
<li>Ability to provide counseling on an individual and group basis to achieve individual treatment plan goals.</li>
<li>Complete discharge summaries</li>
<li>Strong interpersonal skills.</li>
<li>Ability to use a computer and applications that are associated with performing basic work tasks.</li>
<li>Pursue professional training and skills development, and practice with respect to all codes of conduct and ethical boundaries.</li>
<li>Provide support duties for other team members or work within other departments as needed.</li>
<li>Provide on-call duties as assigned.</li>

Other Requirements:

<li>(a) An alcohol and drug counselor must either be licensed or exempt from licensure under chapter 148F.</li>
<li>(b) An individual who is exempt from licensure under chapter 148F, must meet one of the following additional requirements:</li>
<li>(1) completion of at least a baccalaureate degree with a major or concentration in social work, nursing, sociology, human services, or psychology, or licensure as a registered nurse; successful completion of a minimum of 120 hours of classroom instruction in which each of the core functions listed in chapter 148F is covered; and successful completion of 440 hours of supervised experience as an alcohol and drug counselor, either as a student or a staff member;</li>
<li>(2) completion of at least 270 hours of drug counselor training in which each of the core functions listed in chapter 148F is covered, and successful completion of 880 hours of supervised experience as an alcohol and drug counselor, either as a student or as a staff member;</li>
<li>(3) current certification as an alcohol and drug counselor or alcohol and drug counselor reciprocal, through the evaluation process established by the International Certification and Reciprocity Consortium Alcohol and Other Drug Abuse, Inc.;</li>
<li>(4) completion of a bachelor's degree including 480 hours of alcohol and drug counseling education from an accredited school or educational program and 880 hours of alcohol and drug counseling practicum; or</li>
<li>(5) employment in a program formerly licensed under Minnesota Rules, parts 9530.5000 to 9530.6400, and successful completion of 6,000 hours of supervised work experience in a licensed program as an alcohol and drug counselor prior to January 1, 2005.</li>
<li>(c) An alcohol and drug counselor may not provide a treatment service that requires professional licensure unless the individual possesses the necessary license. For the purposes of enforcing this section, the commissioner has the authority to monitor a service provider's compliance with the relevant standards of the service provider's profession and may issue licensing actions against the license holder according to sections 245A.05, 245A.06, and 245A.07, based on the commissioner's determination of noncompliance.</li>

Basic Function

<ul>
<li>Participate in the development and implementation of an individualized patient care plan for the resident with allied health team members.</li>
<li>Implement total nursing care plan through assessment, planning and evaluation.</li>
<li>Administer prescribed medications and treatments according to policy and procedures, evaluate treatment effectiveness on a continuing basis.</li>
<li>Assist in maintaining a physical, social and psychological environment, which is conducive to the overall welfare of the resident.</li>
<li>Participate in resident and family teaching.</li>
<li>Monitor performance of team members and become involved in staff development, evaluation and discipline when indicated.</li>
<li>Assign nursing care duties to team members and ensure such duties are completed in an appropriate manner.</li>
<li>Remain current in facility policies, procedures and nursing trends by participating in inservice and continuing education programs.</li>
<li>Recognize significant changes in the condition of residents and take necessary action.</li>
<li>Document nursing care rendered resident response and all other pertinent and necessary data as outlined in facility's policies and procedures</li>
<li>Function as a charge nurse by serving as a resource person to staff, answer questions, problem solve, and make decisions relevant to resident care.</li>
<li>Adhere to all facility and department safety policies and procedures.</li>
</ul>
Supervision Received
Receives supervision from the Director of Nursing.
Qualifications
<ul>
<li>Graduation from an accredited nursing education program.</li>
<li>Wisconsin registration as a professional nurse.</li>
<li>Requires employee to have had no abuse convictions.</li>
</ul>
Knowledge, Skills, and Abilities
<ul>
<li>Ability to communicate and work well with patients and staff members</li>
<li>Ability to follow facility policies and procedures.</li>
<li>Ability to speak the English language in an understandable manner.</li>
<li>Ability to see and hear or use prosthetics that will enable these senses to function adequately to assure that the requirements of this position can be fully met.</li>
<li>Ability to relate to and work with the ill, disabled, elderly, mentally ill, emotionally upset and at times hostile and violent residents within the facility.</li>
<li>Ability to remain current in practice as applicable to job.</li>
