JOB SUMMARY

As a Home Coordinator, you will be responsible for the supervision, guidance, and training of Direct Support Professionals. This role will also require you to provide quality support services to clients served by Buckeye Residential Solutions. You will be responsible to ensure the homes you oversee are compliant, clean, and safe. In this position, you are a critical member of the team as you will have direct accountability for your staff and clients. You will also be expected to maintain accurate records as required by Buckeye Residential Solutions and any other governing agency such as DODD, ODHS, and County Case Management.
PRIMARY RESPONSIBILITIES

Employee Supervision/ Training

<ul>
<li>Train new staff/transferring staff on client specifics, which include the ISP, BSP and medication specific training. Ensure staff is cross-trained at all houses for which you are responsible.</li>
<li>Maintain strong communication within your homes to ensure all employees understand their roles and responsibilities.</li>
<li>Recommend and participate in discipline as needed for your direct reports.</li>
<li>Assist in developing employees for growth opportunities.</li>
<li>Attend and participate in scheduled staff meetings, designated training and in-service sessions.
<ol>
<li>Lead monthly House Meetings for each of your sites.</li>
</ol>
</li>
<li>Serve in an on-call rotation.</li>
<li>Schedule Residential staff in accordance with business needs and company policy.</li>

Client Care
<ul>
<li>Solve essential client issues and escalate irregular problems to the Residential Program Coordinator along with proposed solutions.</li>
<li>Carry out individual service plans (ISP) including all goals, training plans and correspondence.</li>
<li>Provide Constant security checks as described below:
<ol>
<li>Alarms in place and working. (As appropriate)</li>
<li>Supervision in place. (As appropriate)</li>
<li>Sharps locked up. (As appropriate)</li>
<li>Meds and/or chemicals locked up. (As appropriate)</li>
</ol>
</li>
<li>Collaborate with the Program Coordinator on behavioral issues concerning clients.</li>
</ul>
<ul>
<li>Report all UIs and MUIs as required by law to Residential Program Coordinator for follow up.</li>
</ul>
<ul>
<li>Assist clients as needed with:
<ol>
<li>Health care and personal hygiene.</li>
<li>Meal Preparations and shopping.</li>
<li>Household tasks such as cleaning and laundry.</li>
<li>Recreation, social and leisure time skills.</li>
<li>Transportation as required.</li>

<li>Medications
<ul>
<li>Pass meds.</li>
<li>Making sure that any new medications are being passed correctly and documented properly on the MAR.</li>
<li>Notifying the Residential Program Coordinator when medications and medical supplies have 7 or less days remaining.</li>

<li>Ensure all of your sites are clean and maintain all needs within the home.</li>
</ul>
Administrative Duties
<ul>
<li>Complete required documentation forms, including but not limited to: homemaker/personal care, incident reporting, skill development, medication logs, communication logs, etc.</li>
<li>Update and complete inventory sheets.</li>
<li>Ensure receipts are turned in and any cash on hand is counted monthly.</li>
<li>Weekly review, and where necessary correction of, all information required for regulatory compliance purposes to ensure adequate documentation.</li>
<li>Ensure that there is a complete first aid kit in the home.</li>
<li>Verify that Medical Forms are in the home and staff have been delegated to take clients to appointments when appropriate and that medical appointment forms are being turned in.</li>
<li>Adhere to confidentiality guidelines, as outlined by organization policy and HIPPA regulations.</li>
</ul>
Other Duties as assigned.
WORKING CONDITIONS:

<li>You will be required to work shifts every other weekend. </li>
<li>Job will require sitting, standing, bending, lifting, pushing, and pulling, intermittently.</li>

QUALIFICATIONS:

<li>18 years or older.</li>
<li>High school diploma or equivalent. </li>
<li>2+ years providing direct care in a home setting.</li>
<li>1+ years of management experience preferred.</li>
<li>Current certifications in: CPR, CPI, MUI, First Aid, Medication Administration, and Documentation Training.

The Prevention Health Navigator is responsible for the implementation of health navigation services at Equitas Health. The Prevention Health Navigator will also conduct client-centered HIV test counseling services to individuals voluntarily seeking HIV testing services both onsite and offsite. This position promotes the independence, stability, and health of persons at high risk of attaining HIV and those living with HIV/AIDS by providing culturally competent health navigation services.

ESSENTIAL JOB FUNCTIONS:
Essential functions of the job include, but are not limited to, planning, organizing, implementing and facilitating group and outreach activities, traveling, driving or having reliable transportation, utilizing a computer for typing and data inputting, online outreach, and conducting research; attending meetings; and presenting to small and large groups.
MAJOR AREAS OF RESPONSIBILITY:

<li>Complete assessments as needed, that focus on specific barriers and assist the client in successfully overcoming barriers to HIV prevention. Prevention Health Navigators will assist clients in completing and submitting all necessary documentation related to these assessments. </li>
<li>Function as a central and primary access point for financial assistance programs, including but not limited to Prevention Assistance Program Interventions (PAPI), Gilead Patient Assistance Program, and other assistance programs as appropriate. Prevention Health Navigators will complete and submit paperwork as is needed to support client in maintaining these assistance programs.</li>
<li>Provide input and support to the prevention and education department.</li>
<li>Keeping informed about current HIV/STI trends, developments, and co-factors to transmission (i.e., AOD, Mental Health, etc.), as well as state-of-the-art prevention efforts and strategies.</li>
<li>Keeping informed about current trends for People who inject drugs (PWID) and harm reduction strategies</li>
<li>Keeping informed about current Intimate Partner violence (IPV) and Domestic Violence (DV) trends within LGBTQ+ communities.</li>

JOB SUMMARY

The LPN will serve as an on-call resource across all counties to our residential and day habilitation clients and employees for non-urgent medical needs and questions. You will collaborate with other members of the leadership team, Service and Support Administrators (SSAs), and guardians. The delegated nurse is expected to make independent nursing decisions as related to the client as well as delegate and coordinate nursing care by providing training/ certifications to staff. You will also be expected to maintain accurate records as required by Buckeye Residential Solutions and any other governing agency such as DODD, ODHS, and County Case Management.
PRIMARY RESPONSIBILITIES

<ul>
<li>Provide appropriate medical training to staff as needed.</li>
<li>Coordinate and facilitate communication to all members of the team, which may include service and support administrators (SSAs), day service representatives, and family or legal guardians.</li>
<li>Adhere to confidentiality guidelines, as outlined by organization policy and HIPPA regulations.</li>
<li>Serve as on-call support as needed for medical guidance and/or questions relating to our clients.</li>
<li>Review med administration records as needed.</li>
<li>Work with Pharmacy and Program Administrators to ensure that all meds are up to date.</li>
<li>Provide Med Re-Certifications as needed.</li>
<li>Perform self-med assessments to individuals as needed</li>
<li>Will correspond with RN oversight as needed</li>
</ul>
Other Duties as assigned.
QUALIFICATIONS:
<ul>
<li>Current license as registered professional nurse in state of Ohio as issued by Board of Nursing.</li>
<li>Strong communication and organizational skills.</li>
<li>Self-motivated with ability to work under little supervision.</li>
<li>Ability to complete and pass a background check per company standards.</li>
<li>Positive attitude with the desire to help others.</li>

Summary;
Responsible for the coordination of the MDS process, based on the comprehensive assessments of the resident's needs.
Essential Duties and Responsibilities include the following. Other duties may be assigned.

<li>Completes initial assessment of residents and all subsequent assessments.</li>
<li>Updates physician's orders as needed.</li>
<li>Responsible for entire MDS process.</li>
<li>Reviews MDS assessments when there is a change in a resident's condition and/or quarterly.</li>
<li>Develops Comprehensive Care Plan with the Interdisciplinary Team.</li>
<li>Makes recommendations for charting requirements.</li>
<li>Informs staff of any changes regarding resident care and care issues.</li>
<li>Monitors "RUG" scores for optimal reimbursement.</li>
<li>May be required to take on-call needs and assist with routine LPN duties.</li>
