Summary
The Licensed Practical Nurse provides care to residents. Responsible for maintaining a safe abuse free environment for all residents in the Orchards of Alliance facilities.
 
Essential Duties and Responsibilities
Teamwork and all other duties and responsibilities assigned.
<ul>
<li>Responsible for the administration of medications and treatments as prescribed by the physician.</li>
<li>Responsible for the oversight and direction of care provided by nursing assistants. Immediately reports substandard care or other performance issues to the Charge Nurse.</li>
<li>Receives and provides verbal report regarding new admissions, discharges, transfers, current resident status and new orders at shift change.</li>
<li>Administers controlled substances according to The Stone Pear Pavilion policy. Complete narcotic count with oncoming licensed staff and verifies inventory by signatures of both licensed staff. Discrepancies are immediately reported to the Charge Nurse.</li>
<li>Maintains accurate documentation regarding resident conditions, medications, treatments and other mandated records.</li>
<li>Maintains knowledge of approved documentation guidelines.</li>
<li>Reports any change in resident condition to Charge Nurse immediately.</li>
</ul>
 
Other Responsibilities
<ul>
<li>Initiates immediate intervention in abusive situations to protect the resident and report allegations according to facility policy and regulatory guidelines.</li>
<li>Attends monthly mandatory nursing in-services.</li>
<li>Functions as a resident advocate to assure that Resident Rights are not violated. Communicates with staff, ancillary departments and family members/responsible part to discuss issues related to the resident’s condition of care.</li>
<li>Maintains safe and comfortable environment by maintaining education of usage of all facility safety devices and pressure relief techniques.</li>
<li>Attends other meetings and training sessions as required or appropriate for position.</li>
<li>Completes documentations of reporting and investigates abnormal occurrences via internal report in a timely manner per facility policy and reports findings to Charge Nurse immediately.</li>

Ability to compute volume, rate, ratio, and percentage in the same situations.
 
Reasoning Ability
Ability to deal with several abstract and concrete variables to define problems, collect data, establish facts, and draw valid conclusions when assessing resident needs and conditions, prioritizing own work and that of subordinates, communicating with family members, and investigating situations that arise from residents, family members, supervisors, and employees. Ability to interpret an extensive variety of written or verbal technical instructions from supervisors. Ability to interpret non-verbal gestures, body movements, and facial expressions as cues to determine residents' needs.
 
Computer Skills
Uses the computer to chart daily resident attendance at programs, update care plans, insert new assessment information, record use of PRN medications, and note physical or behavior changes. Uses e-mail to communicate with others internally and externally. Individual should also have knowledge of spreadsheet and word processing software for creating reports and correspondence.
 
Certificates, Licenses, Registrations
Licensed Practical Nurse licensure, CPR certification, and First Aid certification. Each requires periodic renewal through re-certification or continuing education.
 
Other Skills and Abilities
Nursing skills specific to geriatric residents are needed. May need additional skills with special-need geriatric residents such as those with dementia, Alzheimer's or those requiring advanced health care. Ability to use lift equipment.

A DAY IN THE LIFE OF A CASE COORDINATOR/GROUP FACILITATOR
The position requires the development of knowledge and skills in the areas of community support, discharge planning, substance use issues, and mental health issues. The position also requires the ability to respond appropriately to crisis or emergency situations, including at least basic training in crisis intervention and handling a difficult client. Job duties include providing and coordinating activities to assist and support clients in gaining access to needed medical, social, educational and other services essential to their progress in treatment/recovery.
 
· Conduct a Case Management Needs Assessment; develop a Case Management Service Plan and update it according to agency policy.
· Monitor and coordinate the delivery of necessary evaluations (psychiatric, medical, vocational, health, etc.) and assessments in a timely manner to ensure that client's needs are adequately identified.
· Assist in the coordination and development of the client's Behavioral Health Service Plan (Individualized Service Plan ISP). The plan shall include services and assistance necessary to achieve specific objectives as well as an explanation of the availability, intensity and duration required of each service. Review and update the BH Service Plan as required by Meridian HealthCare policy, OMHAS standards and as additional needs and goals are identified. 
· Provide ongoing one-to-one case management services, individual counseling and/or group counseling according to OMHAS standards.
· Provide support to family members and significant others, which includes family education and consultation.
· Assist the client in the development of interpersonal and coping skills, including adapting to home, school, and work environment. Monitors client symptoms and assists the client in self-management of symptoms which interfere with daily living, financial management, personal development, or school or work performance.
· Assist in developing formal and informal community support on behalf of a client or group of clients.
· Assist the client in the procurement and maintenance of essential community resources, including housing and other basis resources.
· Assist the client in increasing social support networks of relatives, friends, volunteer organizations that mediate life stresses resulting from the person's disability. If medically necessary, accompany the individual to activity sites and assist in daily living activities.
· Complete all documentation in a timely manner in accordance to the outlined Agency policy and OMHAS standard and provide necessary data for statistical and billing purposes.

GENERAL STATEMENT OF DUTIES:Under the supervision of the Nursing Manager and other agency physicians and to the extent permitted by Federal and State regulations, the primary functions of this position are to handle and dispense medication, manage the Central Pharmacy function, screen and handle any questions and concerns from clients regarding medical issues, assist the psychiatrists and Nurse Practitioners in their roles, and assess physical health concerns of clients.
 
ESSENTIAL RESPONSIBILITIES:
1. With specific medical authorization and orders from the prescribing physician, administer/dispense a variety of medications to clients. Check behavior and record necessary vital signs and reactions.
2. Assist agency physicians through the observation and recording of client behavior, condition, and response to medication.
3. Maintain, order, record, and store prescribed medications and other medical supplies.
4. Provide emergency care and first aid services as needed.
5. In absence of a physician, assess clients during crisis regarding medication and health issues and take appropriate action.

5.Complete all documentation in a timely manner according to FCS policy. 
6.Communicate in a timely and accurate manner all client-related issues and concerns to all other staff providing service to the client to ensure coordination of care.

Requirements:
ESSENTIAL FUNCTIONS OF THE JOB

<li>Evaluate Patients for rehabilitation services within 24 to 48 hours of physician’s orders.</li>
<li>Develop effective treatment plans.</li>
<li>Treat patients according to the treatment plan of care.</li>
