Position Overview:
<ul>
<li>Provide on-site, team-oriented primary care to patients within Assisted Living and Memory Care communities</li>
<li>Full-time providers care for patients Monday-Friday with admin time built into schedule</li>
<li>Manage a panel of 200 patients (approximately 12-16 patients per/day with 14 being the avg.)</li>
<li>Ability to design and manage your schedule&nbsp;</li>
<li>Develop and implement care plans appropriate to patients and their desires</li>
<li>Clinical support from Clinical Assistants and Care Coordination through LPNs, RNs and Social Workers and care navigators</li>
<li>Collaborate with community partners within home care, hospice, and other resources as appropriate</li>
</ul>
Qualifications:&nbsp;
<ul>
<li>Graduate of an accredited Nurse Practitioner Education Program or Physician Assistant Education Program</li>
<li>Board Certified/Board Prepared through AANP, ANCC, or NCCPA&nbsp;</li>
<li>Licensed or ability to obtain licensure through the Florida&nbsp;Board of Nursing or Board of Medical Practice</li>
<li>Ability to obtain and /or maintain DEA certification for the State of Florida</li>
<li>Proven ability to communicate effectively with strong verbal skills</li>
<li>Excellent interpersonal and customer service skills</li>
<li>Must have a valid and unrestricted driver’s license and minimum state-required liability auto insurance; local travel is required</li>

<li>Licensure &amp; Certification: Active, unrestricted state dental license and CPR/BLS certification.</li>
<li>Preferred Skills: Experience with mobile dental equipment, military experience a plus, and computer proficiency.</li>
<li>Availability: Participation is flexible and completely up to you—choose events that fit your schedule, with a focus on weekend availability to support military readiness</li>
</ul>
