Overview
Registered Dental Assistants perform a variety of patient care, office, and laboratory duties. The primary role of the Registered Dental Assistant (RDA) is to assist the dentist in gaining the patient’s commitment to the best possible treatment, with urgency. The Helpline is open Monday-Thursday 11 AM to 8 PM ET and Friday 11AM 6 PM ET. CSH Navigators will work as needed during the daytime shifts of 11AM to 5PM ET. ESSENTIAL FUNCTIONS:

<li>Provide individual and/or group Outpatient Restoration Services (Education and Case management) with clients that have been deemed incompetent by the Court.</li>

Case Management
<ul>
<li>Coordinate and navigate social service systems such as Medicaid/Medicare, social security, housing supports, Medicaid waivers and CDHS.</li>
<li>Maintain thorough documentation of all service-related contacts.</li>
<li>Develop service and transition plans in collaboration with clients.</li>
<li>Provide community-based services in natural settings, supporting clients in real-life environments.</li>
<li>Connect clients with necessary resources and assist with obtaining such resources. (PCP, dentist, legal support, vocational support)</li>
</ul>
 
Housing
<ul>
<li>Assist clients with navigating housing systems, including completing applications for subsidized housing, transitional housing, or supportive housing programs.</li>
<li>Collaborate with team members to identify barriers to housing, such as credit issues, criminal history, or lack of documentation, and develop plans to address them.</li>
<li>Provide accompaniment to housing appointments or inspections, when necessary, to support client self-advocacy and engagement. </li>
</ul>

Required Knowledge &amp; Skills

<li>Strong written and verbal communication skills.</li>
<li>Understanding of HIPAA, confidentiality, and protected health information.</li>

<li>Commitment to cultural awareness, equity, and working with diverse populations.</li>
<li>Strong conflict resolution and decision-making skills.</li>
<li>Experience working or collaborating with the judicial system</li>
<li>Understanding of the full cycle of care from initial intake through discharge, including developing care plans.</li>

Qualifications
Education

<li>Bachelor's degree in Social Work, Psychology, or a related behavioral health field (required).</li>
</ul>
 
Experience
<ul>
<li>Minimum one year of relevant experience in behavioral health and/or case management (required).</li>
<li>Experience working with individuals with co-occurring mental health and substance use conditions</li>
<li>Experience with housing navigation is a plus</li>
<li>Spanish-speaking/bilingual candidates preferred. </li>
