Essential Functions &amp; Responsibilities
<ul>
<li>The physician serves as the attending physician for the residents residing at each facility;</li>
<li>The physician collaborates with Nurse Practitioners and Physician Assistants to manage day to day resident care at each facility. Typically, each facility will have a full-time PA or NP assigned to the facility depending on the acuity and volume of the patient population;</li>
<li>Physician will provide high quality, patient-centered, comprehensive primary care services by rounding on patients residing in SNFs;</li>
<li>Physician is responsible for diagnosing and treating patients with acute illness, such as bacterial and viral infections, as well as chronic diseases and conditions, such as hypertension, heart disease, or diabetes</li>
<li>Provider documentation is completed in Theoria's proprietary electronic health record (EHR) system, ChartEasy™</li>
<li>PointClickCare, MatrixCare, and American HealthTech are the most commonly used facility EHR systems</li>
<li>Participating in Theoria's Chronic Care Management, Transitional Care, RPM and other medical quality initiatives and programs</li>

Shift Structure

Job Description 
We are seeking a passionate and dedicated Physical Therapist to join our team at ARC Physical Therapy+ in Lee's Summit. As a vital member of our clinic, you will play a key role in delivering high-quality, patient-centered care and supporting individuals throughout their recovery journey.
<ul>
<li>Assess patients to develop and initiate treatment plans based on the evaluation results</li>
<li>Review the physician's referral (prescription) and the patient's condition and medical records to determine the physical therapy treatment required</li>
<li>Provide direct patient care by established protocols and patient care guidelines</li>
<li>Document findings, progress, and instructions to patients and caregivers</li>
<li>Responsible for patient care performed by assistants and rehabilitation technicians</li>
<li>Complete a written plan of care, develop goals based on evaluation findings, and develop a comprehensive program to attain goals</li>
<li>Implement physical therapy treatment program and provide essential physical therapy treatments</li>
<li>Communicate effectively with patients/caregivers and professional colleagues, including physicians and insurance companies</li>
<li>Ensure timely documentation, including daily notes, weekly workers' compensation reports, progress reports, initial evaluations, discharge summaries, and incident reports</li>

Qualifications 
<ul>
<li>Graduate degree from CAPTE-accredited Physical Therapy program.&nbsp;New graduates are encouraged to apply, we’d love for you to start your career with us!</li>
<li>Current state of Missouri&nbsp;license and CPR certification</li>
<li>Ability to work efficiently/effectively in an autonomous environment</li>
<li>Basic PC skills and knowledge, including but not limited to Electronic Medical Record (EMR) system, database software, and MS Office</li>

<li>$75,000-$90,000 - Based on experience</li>
<li>Monday - Friday schedule (8:00 AM- 5:00PM)</li>
