About Chloe House
Chloe house is a Pre and Post- Transitional Care Facility for adolescent girls referred by the DC Department of Youth Rehabilitation Services. Our mission is to provide a supportive environment while fostering relationships with our community partners to ensure successful and positive outcomes for our residents.
Job Summary:
The Case Manager will report to the Program Manager of the Chloe House. They will provide support to clients and work with relevant staff to assess and identify needs and appropriate services to ensure success of the program. The schedule for this position is 2:30 pm - 10:30 pm Monday- Friday.
Qualifications
- License/Certification: Bachelor’s Degree in Social Work, Counseling, psychology, or another relevant Human Services field preferred OR 3 experience in the human service field preferably working with adolescents and/or young adults required;
- A minimum of 2 years of extensive experience with at risk youth;
- Competency in crisis intervention and de-escalation;
- Strong communication and organizational skills required; and,
- This is a full-time, exempt level position. All candidates must possess a valid driver’s license and have reliable transportation; pass the required clearances; and provide any additional information regarding previous records, if necessary.
Duties and Responsibilities
- Completes all necessary funder specific admission processes with clients when referred to the Therapeutic Group Home including intake, program rules and policies (confidentiality, bullying, release of information, etc.).;
- Provides support in creating, implementing and monitoring the Individual Treatment and Behavior Modification and Management Plans;
- Supports the Linkage to Care Coordinator in connecting clients with appropriate social services;
- Conducts individual counseling sessions with clients on caseload;
- Supports the reporting, planning and implementation of activity schedule at the program including various life skills, workshops and trainings on relevant topics to clients;
- Support or provide crisis intervention and on-site counseling when appropriate;
- Perform regular house check inspections inside the building and in the area immediately outside; monitor facilities and enforce program rules for client safety;
- Help provides office coverage, answer and record all phone calls and visitors;
- Complete service documentation in client case records within 24-48 hours of the service; maintain accurate, complete, and confidential case files, written documentation, reports to track client progress and outcomes
- Participate in regular biweekly team consultation meetings with the Clinician, Behavior Modification Technician, Linkage Care Coordinator and other staff working with shared clients;
- Complete 40 hours of training annually;
- Participate in staff meetings, conferences, regular supervision and community outreach events as required;
- Other duties as assigned; and,
- Must have a commitment to ending youth homelessness, racial equity, inclusion, and issues central to Sasha Bruce Youthworks mission.
