Key Responsibilities:

<li>Ensure the completion of patient databases, treatment plans, progress notes, discharge summaries, and other essential documentation in accordance with applicable federal and state standards and agency policies and procedures.</li>
<li>Provide patients with orientation regarding objectives of the program, phase system requirements, rules and regulations, sanctions, hours of services, patient rights, and additional information to help patients adjust to the program.</li>
<li>Assure each patient is assessed and placed according to the treatment phases in accordance with Federal and State standards and agency policies.</li>
<li>Assure all clinical documentation and patient charts updated in accordance with established policies and procedures.</li>

Responsibilities
<ul>

Summary:
A Home Health Registered Nurse (RN) delivers direct nursing care to patients in their homes, ensuring comfort, stability, and effective management of their medical conditions. Essential Work Functions:

<li>Assess patient conditions and develop individualized care plans based on medical needs and physician directives</li>
<li>Administer medications, treatments, and wound care while monitoring patient responses</li>
<li>Provide patient and caregiver education on medication management, disease progression, and self-care techniques</li>
<li>Monitor for changes in patient status, reporting concerns and adjusting care plans as needed</li>
<li>Coordinate with physicians, therapists, and other healthcare professionals to ensure seamless care transitions</li>
<li>Document patient care, interventions, and progress accurately and in compliance with regulatory standards</li>
<li>Ensure compliance with infection control, safety, and home care best practices to protect patients and caregivers</li>
<li>Support families and caregivers, offering guidance and emotional support throughout the care process</li>
<li>Maintain communication with case managers and interdisciplinary teams to provide coordinated patient care</li>
<li>Complete charting and documentation requirements while meeting visit and productivity goals</li>
<li>Adhere to infection control, safety, and healthcare regulations</li>
<li>Perform other duties as assigned within the scope of practice</li>

Required Essential Skills:

<li>Technical/Motor Skills - Must have the ability to grasp, perform fine manipulation, push/pull, and move about when assisting with procedures and/or using department equipment</li>
<li>Mental Requirements - Must be able to cope with frequent contact with the general public and customers while meeting deadlines under pressure. Must be able to work under close supervision occasionally, as well as working without assistance from other personnel. Must be able to contend with irregular activity schedules occasionally and continuous concentration to detail</li>
<li>Sensory - Must possess visual acuity and ability to effectively communicate</li>
