Position Summary
The Connection Center Intake/Access Case Manager position supports the provision of Coordinated Entry of social services in Colorado by providing information, referral, intake assessment, and emergency casework services over the phone, via email, video chat, and/or in person. The Connection Center Case Manager will provide case management services using homeless diversion and prevention strategies and will also be responsible for using HMIS and Salvation Army databases to document assistances, referrals, and to track assisted households monthly, for at least one year. Provides easy access to tailored solutions for families in Metro Denver that are experiencing homelessness, are at risk of experiencing homelessness, or who are precariously housed, to promote long term housing stability. The Connection Center is open M-F from 7:00 am to 9:00 pm, and weekends from 8:00a to 12:00pm. Please note that these hours be extended in cold weather activation.
General
· Answers calls for information regarding The Salvation Army’s assistance programs and other community resources.
· Focus on triaging, VISPDATs, and providing assistance as needed.
· Assess needs and identify resources to establish individual case plan.
· Connect qualified candidates with their appropriate partner agency.
· Provide tailored case management and financial assistance when appropriate.
· Documents calls accurately utilizing a computerized internal database.
· Record and track assistances in HMIS according to HUD standards.
· Completing all required assessments with households to determine program eligibility.

Education/Experience

The Chief Medical Officer (CMO) is accountable for CINQCARE’s clinical strategy, operational performance, and health outcomes across all markets.
The CMO provides leadership, oversight, and direction to ensure CINQCARE delivers safe, high-quality, equitable, and cost-effective care. The CMO will drive the design and execution of care models, clinical operations, and value-based care initiatives, while also partnering across functions to achieve enterprise growth and impact.

Key Responsibilities
Leadership &amp; Strategy
<ul>
<li>Provide visionary leadership for CINQCARE’s clinical enterprise, ensuring alignment between strategy, operations, and quality outcomes.</li>
<li>Serve as the principal medical leader responsible for defining, implementing, and optimizing CINQCARE’s care models and clinical pathways.</li>
<li>Lead P&amp;L accountability for CINQCARE’s Care Pathway programs (e.g., CINQCARE 4 Moms, CINQCARE’s Cognitive Impairment programs, including CMS GUIDE Dementia Program).</li>
<li>Partner with Business, Clinical Operations, Network, and Regional leadership to deliver measurable outcomes in access, quality, and cost.</li>
<li>Represent CINQCARE to external stakeholders, including payers, community organizations, government entities, and regulatory agencies.</li>
<li>Establish enterprise-wide standards of performance for clinical quality, operational efficiency, and Family Member satisfaction.</li>
<li>Build and sustain a high-performing clinical leadership team focused on operational excellence and care innovation.</li>
</ul>
 Clinical Excellence &amp; Quality
<ul>
<li>Oversee the design, implementation, and continuous improvement of CINQCARE’s clinical protocols, standards, and guidelines.</li>
<li>Ensure compliance with all federal, state, and local healthcare laws, regulations, and accreditation standards.</li>
<li>Lead quality improvement initiatives to ensure patient safety, care coordination, and evidence-based practice.</li>
<li>Drive integration of clinical and social determinants of health into care models for holistic Family Member outcomes.</li>
<li>Utilize data analytics to monitor performance, reduce clinical variation, and manage total cost of care.</li>
<li>Chair and guide Quality Assurance, Credentialing, and Clinical Governance Committees.</li>
</ul>
 Growth &amp; Network Development
<ul>
<li>Partner with the CEO and Executive Leadership to execute CINQCARE’s strategic growth agenda across existing and emerging markets.</li>
<li>Recruit, engage, and retain high-performing physicians and advanced practice providers through employment, IPA, and ACO partnerships.</li>
<li>Support provider alignment strategies that enhance CINQCARE’s ability to deliver culturally responsive, community-based care.</li>
<li>Lead development of new service lines, programs, and population health initiatives that address the needs of high-needs, urban, and rural communities.</li>
<li>Drive innovation in care delivery models, leveraging technology, data, and partnerships to expand access and equity.</li>
</ul>
 Operations &amp; Financial Management
<ul>
<li>Oversee operational performance for CINQCARE’s clinical programs and ensure adherence to budgetary and performance goals.</li>
<li>Collaborate with Finance and Network Operations to manage productivity, utilization, and clinical spending.</li>
<li>Develop operational plans, policies, and internal controls that ensure accountability and alignment across all clinical sites.</li>
<li>Partner with Technology and Analytics teams to integrate actionable insights into operational decision-making.</li>
<li>Maintain oversight of physician compensation, contracting, and value-based incentive structures to promote efficiency and high-quality care.</li>

Required Qualifications

<li>M.D. or D.O. from an accredited U.S. medical school required; advanced business degree (MBA, MHA, or MPH) strongly preferred.</li>

The working environment and physical requirements of the job include:
In-office work is performed indoors in a traditional office setting with conditioned air, artificial light, and an open workspace.
In this position you will need an to communicate with customers, vendors, management, and other co-workers in person and over devices, sometimes with people who are agitated. Regular use of the telephone and e-mail for communication is essential. Sitting for extended periods is common. Must be able to receive ordinary information and to prepare or inspect documents. Job Description
We are seeking a dedicated full-time Clinical Therapist to join our behavioral health team specializing in substance use disorder (SUD) and mental health treatment with mostly justice involved clients. This position is responsible for providing comprehensive assessments, individualized treatment planning, therapeutic interventions (individual and group therapy), and case coordination for clients struggling with substance use and co-occurring mental health disorders. The ideal candidate will have experience in addiction treatment, an understanding of trauma-informed care, experience providing couples therapy, experience with justice involved clients, and licensure (or license eligible) to practice in the state of Colorado.
Key Responsibilities
<ul>
<li>Conduct substance use and mental health assessments using ASAM criteria and other validated tools.</li>
<li>Develop, implement, and regularly update individualized treatment plans that address substance use and any co-occurring behavioral health needs.</li>
<li>Provide individual therapy, assessments, couples therapy, and 4-5 groups a week.</li>
<li>Therapist would be involved in providing services in community correction facilities.</li>
<li>Monitor client progress, update documentation in compliance with agency, state, and federal standards, and maintain confidentiality in accordance with HIPAA and 42 CFR Part 2 regulations.</li>
<li>Create monthly reports for all justice involved clients to send to referring/supervising agencies. </li>
<li>Collaborate with case managers, peer support specialists, medical professionals, referring agencies, and external service providers to ensure coordinated care.</li>
<li>Participate in clinical supervision, staff meetings, training, and quality improvement initiatives.</li>
<li>Assist in crisis intervention and safety planning as needed. </li>

Education &amp; Licensure:

<li>Master's degree in counseling, Social Work, Psychology, or a related behavioral health field from an accredited program. </li>
<li>Must possess a valid Colorado clinical license (LPC, LAC, LCSW, LMFT, or related) or be license eligible (e.g. LPCC). </li>
