 Position Summary: We are seeking a Licensed Practical Nurse (LPN) to provide patient-centered care in a home health setting. The LPN will deliver services in accordance with the Connecticut Nurse Practice Act, agency policies, and professional standards of care, under the direction and supervision of a Registered Nurse. Qualifications:

<li>Support patients in achieving optimal health outcomes and independence</li>

<li>Use proper EVV clock-in/out procedures for accurate visit tracking</li>

You will also complete Disability Benefits Questionnaires (DBQs) through the Web Portal.
The Licensed Psychologist DOES NOT make disability determinations. The U.S. Veterans Administration makes all disability determinations, in part using the findings recorded in the DBQs.
This role ONLY performs Disability Assessments on Veterans. You will NOT treat, and you will NOT prescribe medication in this position. 
Licensed Psychologist will review and assess 4 Veterans per day on average.
Licensed Psychologist will be brought on as an Independent Contractor
Travel requirements:
We would need you to commit to at least two weeks of travel each month.
Travel to assigned location Sunday evening, return to home city Saturday morning.
