Center for Life Management (CLM) provides the area's most comprehensive array of outpatient mental health services for adults and elders. Our multi-disciplined treatment team works with individuals ages 18+ who experience severe and persistent mental illness and/or co-occurring substance use disorders. The Adult Case Manager provides care planning, resource identification, benefit coordination, service monitoring, and community-based support to assist clients with improving symptoms and behaviors that otherwise interfere with daily functioning and wellness goals. This is a full time, 40 hour per week position.
Major Functional Areas and Description of Duties
<ul>
<li>Provide individual assessment to create care plans that specify presenting problems, objectives, interventions, and desired outcomes;</li>
<li>Assist clients with applying for and maintaining adequate medical coverage to support consistent access to whole health care and treatment;</li>
<li>Provide outreach, advocacy, coordination, resource identification, linkage and monitoring of internal and external supports and services;</li>
<li>Provide counseling in the home or other community-based setting as deemed appropriate for the objectives, interventions and desired outcomes identified in the client’s treatment plan;</li>
<li>Develop, apply, and reinforce coping strategies to reduce and/or ameliorate symptoms and behaviors that interfere with functioning and goal achievement;</li>
<li>Provide crisis supports when clients experience acute exacerbation of symptoms to ensure their safety within the home and/or community;</li>
<li>Provide timely documentation on each service delivered;</li>
<li>Work collaboratively with other treatment team members, natural supports, and external providers to ensure a wrap-around approach is provided for continuity of client care;</li>
<li>Provide set case management contacts per month and functional support services as needed to meet client care (eligible for incentive pay upon exceeding productivity expectation);</li>
<li>Serve on appropriate committees, both internal and external, as determined by supervisor;</li>
<li>Function professionally and in a manner that protects the integrity, confidentiality and rights of all patients;</li>
<li>Execute all required services and documentation in accordance with agency policies and federal/state standards;</li>
<li>Maintain professional memberships, licenses, accreditation, certifications, etc.;</li>
<li>Represent CLM in a professional and appropriate manner in all settings;</li>
<li>Perform all other duties as assigned by supervisor.</li>
</ul>
Qualifications and Skills
<ul>
<li>Bachelor’s degree in a health related field preferred (ex. Social Work, Mental Health Counseling, Psychology, Nursing, Human Services, etc.);</li>
<li>Associate’s degree in a health related field will be considered, along with a minimum of two years of direct service experience;</li>
<li>Case management or case coordination experience preferred;</li>
<li>Ability to exercise sound clinical judgment in routine, urgent, and emergent situations;</li>
<li>Proficient computer skills, specifically with all Microsoft office applications;</li>
<li>Ability to document effectively and efficiently in course of clinical work;</li>
<li>Ability to navigate an electronic health record;</li>
<li>Ability and desire to work as part of a clinical team while also able to be self-directed and motivated with minimal supervision;</li>
<li>Ability to acquire and effectively practice new clinical skills as needed according to evolving agency needs/services;</li>
<li>Effective communication skills, organizational skills, and ability to multi-task.</li>
</ul>
Physical Demands and Working Conditions
The majority of the work will be office based and/or remote via telehealth. Some of the work will be visiting clients in the community and also in their homes. Manual dexterity is needed to use computers. The ability to sit for extended periods of time is required. Reliable transportation, a clean driving record, and proof of auto insurance coverage is required.

Basic Qualifications

<li>BS in Pharmacy or Pharmacist Degree from an accredited educational institution.</li>
<li>Current pharmacist licensure in the states within the district. </li>
<li>Experience performing prescription dispensing activities that demonstrate a strong working knowledge of applicable state and federal controlled substance laws. </li>
<li>Certified Immunizer or willing to become an immunizer within 90 days of hire.</li>

 We are looking for a Patient Care Coordinator to join our Orthopedic practice. The primary responsibility is to answer incoming phone calls from patients to make an initial appointment and process incoming referrals to the clinic. The Patient Care Coordinator works closely with the triage nurse and follows physician protocols and insurance guidelines. Follow-up appointments may be scheduled and completes paperwork as needed. Performs other related duties as assigned. Minimum Qualifications:

<li>High school diploma or equivalent required.</li>
<li>Knowledge of medical and insurance terminology, billing and coding experience is a plus</li>
<li>One year of prior surgery scheduling in a healthcare setting required</li>
<li>Knowledge of HIPAA practice procedure.</li>
<li>Experience with EHR</li>
<li>Efficient in the use of standard office equipment, data entry, typing, 10-key skills, and alpha/numerical filing.</li>

 Primary Responsibilities

<li>Schedule new patients as they call/contact through website/patient portal/fax within 48 hours of initial contact.</li>
<li>Reach out to patients upon receipt of referral via fax within 48 hours.</li>
<li>Request and track medical records from primary/referring physicians.</li>
<li>Follow-up with patients where messages were left. Document attempts</li>
<li>Coordinate with Triage nurse and provider to review (consult) medical records for potential procedure patients.</li>
<li>Contact referring physician if unable to reach patient after 3 attempts or patient declines appointment. Document both conversations.</li>
<li>Verify insurance coverage, including benefits and eligibility.</li>
<li>Assist co-workers at the direction and discretion of supervisor/administrator.</li>
</ol>
