The following duties are typical for the Maternal Care Coordinator. Incumbents may not perform all of the listed duties and/or may be required to perform additional or different duties from those set forth below to address business needs and changing business practices.
 
Care coordination:

Specifics for Maternal Health:

<li>Manages a caseload of prenatal patients. Coordinates services and provides case management.</li>
<li>Facilitates education on infant care, breastfeeding, and postpartum health.</li>
<li>Ensure patients complete required prenatal labs and obstetrical imaging. </li>
<li>Assists patients post-delivery by obtaining hospital birthing records, recording Apgar scores, and birth weights, assisting with FLMA and maternity leave paperwork, and conducting follow-up outreach to encourage postpartum appointments for 1 year postpartum. </li>
<li>Facilitates referrals for clinically indicated services outside of HKH.</li>
<li>Conducts outreach and supports patients, including home visits. </li>
<li>Track the maternal patients in the AZARA OB module. </li>
<li>Performs other duties as assigned.</li>

Job Summary and Qualifications
 
Under the supervision of an RN, the LVN staff nurse provides care in accordance with policies and procedures. In collaboration with the patient, the patient’s family and other members of the health care team, the clinical LVN works under the supervision of a registered nurse in the delivery of patient care.
