CLM provides the area's most comprehensive array of mental health services for children and their families. Our multi-disciplined treatment team works with individuals ages 15-26 who experience mental illness, serious emotional difficulties and first episode psychosis. The Young Adult Community Case Manager provides care planning, resource identification, benefit coordination, service monitoring, and community-based counseling support to assist clients with improving symptoms and behaviors that otherwise interfere with age-appropriate development, functioning and wellness goals. This is a full time, 35 hour per week position and is considered a non-exempt, in accordance with the Fair Labor Standards Act (FLSA).
 
Major Functional Areas and Description of Duties

<li>Provide individual assessment to create care plans that specify presenting problems, objectives, interventions, and desired outcomes.</li>
<li>Assist clients with applying for and maintaining adequate medical coverage to support consistent access to whole health care and treatment.</li>
<li>Provide outreach, advocacy, coordination, resource identification, linkage and monitoring of internal and external supports and services.</li>
<li>Provide counseling and Functional Support Services (FSS) in the home, school or other community-based setting as deemed appropriate for the objectives, interventions and desired outcomes identified in the client’s treatment plan.</li>
<li>Provide Individual Therapeutic Behavioral Services (TBSI) to develop, apply, and reinforce coping strategies to reduce and/or ameliorate symptoms and behaviors that interfere with functioning and goal achievement.</li>

<li>Provide Crisis Intervention (CI) when clients experience acute exacerbation of symptoms to ensure their safety within the home and/or community. </li>
<li>Provide timely documentation on each service delivered.</li>
<li>Work collaboratively with other treatment team members, natural supports, and external providers to ensure a wrap-around family systems approach is provided to the client.</li>
<li>Provide set case management contacts per month andsethours of functional support services per week (eligible for incentive pay upon exceeding productivity expectation). </li>
<li>Serve on appropriate committees, both internal and external, as determined by supervisor.</li>

Physical Demands and Working Conditions
Half of the work will be visiting clients in the community and also in their homes. The other half will be office based. The ability to actively engage with children and conduct interactive play within a therapeutic setting is required. Manual dexterity is needed to use computers. The ability to sit for extended periods of time is required. Reliable transportation, a clean driving record, and proof of auto insurance coverage is required.

On Behalf of our client we are looking for a dedicated Practice Manager to oversee the daily operations of their doctors office clinic near Prospect Heights , Illinois. This role is essential in ensuring seamless workflows, supporting the team, and maintaining high standards of patient care. The ideal candidate will have the ability to lead with confidence, optimize processes, and foster a collaborative and positive environment.
 
Compensation: $75k-$85k + Bonus
Client only offers PTO, and holiday pay 
(Do not offer Medical, Dental, Vision, or 401k)
Hours of operation: Monday, Tuesday, Thursday: 9:00am-5:30pm
Wednesday, Friday 9:00am-2:00pm
 
Responsibilities:
• Supervise and manage daily office operations, including scheduling, staffing, and inventory management.
• Ensure compliance with healthcare regulations and internal policies.
• Oversee billing processes and medical insurance verifications.
• Collaborate with clinical leadership to drive growth initiatives and improve patient care.
• Cultivate a supportive and mission-driven team culture to promote staff satisfaction and patient outcomes.
• Maintain a consistent, high-quality experience across all patient interactions.
• Ensure the availability and proper management of office supplies and equipment.
• Handle receptionist duties and provide support for administrative tasks as needed.
• Address operational challenges with proactive problem-solving and decision-making.
