SUMMARY:
The Intake Specialist is responsible for the screening and placement of children in appropriate placements and ensuring vacancies are filled. &nbsp;
REQUIRED QUALIFICATIONS:
· A Bachelor’s degree in Social Work or related field.
· 2+ years casework experience in the child welfare field or related field required.

RESPONSIBILITIES:
· Review all referrals for placements from multiple sources

<li>Processes and checks all medications, supplies and equipment prior to dispensing from the pharmacy. </li>
<li>Maintains pharmacy records in accordance with applicable laws and regulations. </li>
<li>Maintains clinical skills and knowledge of pharmacotherapy by reviewing the literature and attending in-services, seminars, and conferences. </li>
<li>Participates in on-call rotation if applicable. </li>
<li>Performs marketing support functions as requested. </li>
<li>Participates in orientation/training programs for new personnel and current staff. </li>
<li>Participates in departmental QA activities. </li>
<li>Attends designated department meetings as scheduled. </li>
<li>Performs other tasks or special projects as requested by management. </li>

<li>Maintain confidentiality of patient and proprietary information at all times. </li>
<li>Observe legal and ethical guidelines for safeguarding the confidentiality of patient and proprietary company information. </li>
<li>Understand and comply with all HIPPA requirements and regulations </li>
<li>Participate in the process for assessing, managing, and reporting medication-related incidents and equipment errors and/or failures. Accurately completes occurrence forms and forwards them to the appropriate governing bodies. </li>
<li>For all activities complies with accreditation, legal, regulatory and safety requirements. </li>
</ul>

Qualifications: (Education, Certification/Licensure and Experience/Skills):
<ul>
<li>A Bachelor of Science in Pharmacy (RPh) or Doctor of Pharmacy (PharmD) from an accredited (Council on Pharmaceutical Education) college of Pharmacy. </li>
<li>Currently Licensed as a registered pharmacist in the state of employment. </li>
<li>Possess a minimum of two years recent experience in an acute-care setting or one-year recent experience in home infusion therapy. </li>
<li>Possess experience and strong knowledge base in intravenous admixture services and intravenous therapy. </li>
<li>Must be capable of assuming responsibility for evaluating and monitoring drug therapy relative to the total needs/problems of the home care patient in collaboration with physicians, nurses, and other healthcare professionals. </li>
<li>Can work effectively with others to provide safe and effective drug therapy. </li>
<li>Can communicate, both verbally and written, with patients, clients, management, and other employees. </li>
<li>Can problem solve and supervise technical and other support staff. </li>
<li>Must be flexible with work schedule. </li>
<li>Possess good organizational and time-management skills. </li>
<li>Participation and membership in professional societies and organizations is desirable. </li>
<li>Dependable in attendance and job performance. </li>
</ul>

Required to stand, walk, sit, talk, and hear; required to use hands to operate vehicles and office equipment; must be able to occasionally lift and/or move up to 25 lbs; specific vision requirements for this job include: close, distance and peripheral vision, and the ability to adjust focus, reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions of this position without compromising patient care.
