Job Title/Position: Registered Nurse (RN)

JOB DESCRIPTION SUMMARY
The registered nurse plans, organizes and directs home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
Patient Care
1. Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
2. Regularly re-evaluates patient nursing needs.
3. Initiates the plan of care and makes necessary revisions as patient status and needs change.
4. Uses health assessment data to determine nursing diagnosis.
5. Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
6. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
7. Counsels the patient and family in meeting nursing and related needs.
8. Provides health care instructions to the patient as appropriate per assessment and plan of care.
9. Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
10. Acts as Case Manager when assigned by Clinical Supervisor/Nursing Supervisor and assumes responsibility to coordinate patient care for assigned caseload.
Communication
1. Prepares clinical notes and updates the primary physician when necessary and at least every sixty days.
2. Communicates with the physician (or other authorized licensed independent practitioner) regarding the patient’s needs and reports any changes in the patient’s condition; obtains/receives physician’s (or other authorized licensed independent practitioner) orders as required.
3. Communicates with community health related persons to coordinate the care plan.
Additional Duties
1. Participates in on-call duties as defined by the on-call policy.
2. Ensures that arrangements for equipment and other necessary items and services are available.
3. Instructs, supervises and evaluates home health aide care provided every two (2) weeks.
POSITION QUALIFICATIONS
1. Registered nurse with current licensure to practice professional nursing in the state.
2. Graduate of National League for Nursing accredited school of nursing.
3. Maintains a current CPR certification.
4. Must be a licensed driver with an automobile that is insured in accordance with state/or
organization requirements and is in good working order.
5. Minimum of two years’ experience, at least one of which is in the area of public health or home care nursing is preferred.
6. Self-directed and able to work with minimal supervision.
7. Demonstrates excellent observation, problem solving, verbal and written communication skills; nursing skills per competency checklist.
8. Shows ability to organize and prioritize workload independently.
9. Management experience not required. Responsible for supervising home health aides.
10. Prolonged or considerable walking or standing. Able to lift, position, or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling, or crouching. Visual acuity and hearing to perform required nursing skills.

Participate in required Sullivan Courses and perform CME credits to maintain license.10. Consult with other physicians on medical care of patients.11. Take patient histories and vital signs as necessary.12. Communicate patient needs, diagnosis, and course of treatment to the patient, company, as well as other clinical staff and instruct and educate patients about discharge instructions if applicable.13. Create an individualized plan of care based on physical, psychosocial and age specific assessments of the patient, as well as patient’s input and history.14. Delegate work appropriately to other caregivers and insures accuracy of delegated work.15. Maintain an adequate patient flow on-site by directing assisting clinical staff.16. Maintain quality assurance of charts and testing.17. Handle telephone calls about services rendered from company accounts, patients, physicians, as well as other staff.18. Provide accurate and timely documentation to West Sacramento Urgent Care staff, insurance carriers and companies.19. Maintain patient records for clinical use, medical records use, billing use, physicians review, etc.20. Serve as a resource person for other clinical staff.21. Key Responsibilities
<ul>
<li>Act as the primary point of contact between the operations manager, backend billing teams, and client practices</li>
<li>Communicate directly with clients to resolve questions or concerns</li>
<li>Review client accounts monthly using billing software to track progress</li>
<li>Spot-check submitted and denied claims for accuracy and follow-up</li>
<li>Identify account or service issues and report to the Operations Manager</li>
<li>Provide proactive suggestions to improve client satisfaction and internal workflows</li>
<li>Ensure client accounts are managed with consistency and attention to detail</li>
</ul>
Qualifications
<ul>
<li>2-4 years of experience in medical billing with emphasis on A/R recovery</li>
<li>Familiarity with CPT, ICD-10 codes, and modifiers (certification preferred, not required)</li>
<li>Strong communication, problem-solving, detailed, and follow-through skills</li>
<li>Proficiency in Microsoft Excel, Word, and Google Sheets</li>
<li>Self-starter who thrives in a fast-paced, team-oriented environment</li>

 JOB DESCRIPTION SUMMARY
The registered nurse plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.
 ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES &nbsp;
Patient Care 1. Completes an initial, comprehensive and ongoing assessments of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). 2. Provides professional nursing care by utilizing all elements of nursing process. 3. Assesses and evaluates patient’s status by: &nbsp; &nbsp; &nbsp; &nbsp;A. Writing and initiating plan of care. &nbsp; &nbsp; &nbsp; &nbsp;B. Regularly re-evaluating patient and family/caregiver needs &nbsp; &nbsp; &nbsp; C. Participating in revising the plan of care as necessary. 4. Uses health assessment data to determine nursing diagnosis. 5. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. 6. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician’s plan of care. 7. Counsels the patient and family in meeting nursing and related needs. 8. Provides health care instructions to the patient as appropriate per assessment and plan. 9. Assists the patient with the activities of daily living and facilitates the patient’s efforts toward self-sufficiency and optional comfort care. 10. Acts as Case Manager when assigned by Clinical Supervisor/Nursing Supervisor and assumes responsibility to coordinate patient care for assigned caseload.
Communication
 1. Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given. Records pain/symptom management changes/outcomes as appropriate. 2. Communicates with the physician regarding the patient’s needs and reports changes in the patient’s&nbsp;condition; obtains/receives physicians’ orders as required. 3. Communicates with community health related persons to coordinate the care plan. 4. Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the interdisciplinary group. 5. Provides and maintains a safe environment for the patient. 6. Assists the patient and family/caregiver and other team members in providing continuity of care. 7. Works in cooperation with the family/caregiver and hospice Interdisciplinary Group Members to meet the emotional needs of the patient and family/caregiver. 8. Attends interdisciplinary group meetings. &nbsp;
Additional Duties
 1. Participates in on-call duties as defined by the on-call policy. 2. Ensures that arrangements for equipment and other necessary items and services are available. 3. Supervises ancillary personnel and delegates responsibilities when required. 4. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes. 5. Fulfills the obligation of requested and/or accepted case assignments. 6. Actively participates in quality assessment performance improvement teams and activities.
POSITION QUALIFICATIONS
 1. Registered nurse with current licensure to practice professional nursing in the state. 2. Graduate of National League for Nursing accredited school of nursing. 3. Maintains a current CPR certification. 4. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order. 5. Registered nurses shall have a minimum of (a) one (1) year of experience as a professional nurse within the last three (3) years OR (b) have a baccalaureate degree in nursing from a program accredited by the National League for Nursing and a current RN license. Minimum of two years’ experience, at least one of which is in the area of public health, home care, or hospice nursing is preferred. 6. Self-directed and able to work with minimal supervision. 7. Management experience not required. Responsible for supervising hospice aides. 8. Demonstrates excellent observation, problem solving, verbal and written communications. nursing skills per competency checklist. 9. Shows ability to organize and prioritize workload independently. 10. Prolonged or considerable walking or standing. Able to lift, position, and/or transfer patients.
