SHIFT AND SCHEDULE

ESSENTIAL FUNCTIONS/PERFORMANCE EXPECTATIONS
• Complete a comprehensive assessment. This includes the initial and ongoing, extensive collection and interpretation of data.

LCSW / LPC- SUD Therapist for Primary Care

<h3>Job Description</h3>
This role offers a meaningful opportunity for a compassionate and motivated professional to provide direct support and counseling to individuals navigating substance use recovery. As an SUD Counselor, you will contribute to a multidisciplinary care team dedicated to promoting health, stability, and long-term recovery for clients. Key Responsibilities
<ul>
<li>Provide individual and group counseling sessions to clients as assigned</li>
<li>Maintain accurate and timely clinical records in accordance with OASAS regulations</li>
<li>Deliver client-centered care by developing and reviewing Recovery Care Plans and Discharge Plans</li>
<li>Assist with client intake, orientation, and case management services</li>
<li>Respond effectively to crisis situations including intoxication, psychiatric issues, or suicidal ideation</li>
<li>Coordinate care with internal and external service providers; conduct outreach to community resources and absent clients</li>
<li>Support program goals and contribute to continuous quality improvement initiatives</li>
<li>Participate in utilization review processes and respond to insurance-related requests as required</li>
<li>Ensure compliance with confidentiality laws and ethical standards, including adherence to NYS CFR 42</li>
<li>Engage and involve family members early in the treatment process when appropriate</li>
<li>Conduct clinical documentation (notes, assessments, plans, summaries) accurately and on schedule</li>
<li>Attend staff meetings, trainings, and committee assignments as needed</li>
<li>Assist in achieving departmental benchmarks for successful completion, engagement, and reduced drop-out rates</li>
<li>Support the agency’s mission and participate in team coverage and optional programming when needed</li>
</ul>
 
Qualifications &amp; Experience
<ul>
<li>High School Diploma or equivalent (required)</li>
<li>CASAC or CASAC-eligible (required)</li>
<li>Minimum of one year of experience working in substance use treatment or a related setting</li>
<li>Knowledge of OASAS regulations and scope of practice standards</li>
<li>Preferred: Bachelor’s or Master’s degree in a behavioral health field; QHP designation</li>
<li>Bilingual ability is a plus</li>
</ul>
