A Day in the Life of a Respiratory Therapist
The Respiratory Therapist acts as a resource person to the other members of the Cardiopulmonary department and the staff of CRMC. In addition, the RT is responsible for daily patient assignments and work performed on each assigned shift.

Here Is What You Will Be Doing:
<ul>
<li>Performs all assigned therapeutic and diagnostic procedures as ordered by a physician including but not limited to: basic respiratory therapy and oxygen therapy including daily oxygen rounds, optiflow titrations, pulse oximetry monitoring to determine oxygen titration needs and arranging for home oxygen therapy. Bronchodilator therapy including protocols and patient assessment. Acts as a backup for the performance of EKGs.</li>
<li>Performs therapeutic and diagnostic procedures for the pediatric floor. Job duties include but are not limited to daily (morning) pulse oximetry and titration, bronchodilator therapy, RSV protocol, asthma protocols and education, and oxygen therapy. Performs therapeutic and diagnostic procedures in the nursery including but not limited to: oxygen hood, nasal CPAP and ventilator management in both invasive and non-invasive modes. Must attend a minimum of 3 normal child births during orientation</li>
<li>Verifies physician order for therapy to assure that the order is complete and appropriate. </li>
<li>Documents appropriately and completely in patient’s record and department records according to department and hospital policies.</li>
<li>Performs arterial blood gas sampling by direct puncture or arterial line. Analyzes ABG and co-oximetery results and makes educated oxygenation and ventilation decisions. </li>
<li>Performs quality control, proficiency testing, maintenance and troubleshooting of blood gas analyzer.</li>
<li>Assists in Cardiopulmonary resuscitation efforts and/or other emergencies that utilize respiratory care procedures for patient care.</li>
<li>Maintains professional growth by presenting internal and/or external in-service programs, pursuing further certification or registries and reading appropriate magazines and journals.</li>
<li>Must attend 80% of department meetings </li>

PRIMARY SCOPE OF SERVICE:
The Physician Advisor collaborates closely with the medical staff leadership, the entire medical staff, all areas of resource management, case management, and utilization management to develop and implement methods to optimize use of hospital services for all patients while also ensuring the quality of care provided. This includes working with hospital leadership in developing care management protocols with physicians and others to optimize length of hospital stay and efficient management of resources, ensuring patients are in the appropriate level of care, supporting documentation, coding improvements and compliance, and monitoring the appropriate use of diagnostic and therapeutic modalities.
GENERAL REQUIREMENTS:
MINIMUM JOB SPECIFICATIONS:
<ul>
<li>Hold and maintain or able to obtain an unrestricted medical license in the state of Wyoming.</li>
<li>Possess or acquires a solid foundation, knowledge, and/or experience in the areas of utilization management, quality improvement, and patient safety.</li>
<li>Familiarity with InterQual and MCG is preferred.</li>
<li>Strong understanding of Medicare Two Midnight Rules</li>
<li>Member of the American College of Physician Advisors (ACPA) .</li>

ESSENTIAL JOB DUTIES AND ACCOUNTABILITIES
LEADERSHIP: 
<ul>
<li>Provide functional leadership for the revenue integrity team Including CDI, Coding and Utilization Review.</li>
<li>Responsible for oversight revenue integrity optimization.</li>
<li>Lead value-based care initiatives for the organization.</li>
<li>Chairs the Utilization Management Team. </li>
</ul>
UTILIZATION REVIEW SUPPORT: 
<ul>
<li>Review medical records of patients identified by case managers or as requested by the healthcare team including physicians to perform quality and utilization oversight.</li>
<li>Perform medical necessity reviews including initial level of care, secondary reviews, and continued stay reviews.</li>
<li>Conduct Peer to Peer discussion with Payor Medical Directors when requested.</li>
<li>Provide regular feedback to physicians and all other stake holders regarding level of care, length of stay, and potential quality issues.</li>
<li>Provide necessary clinical education to UR Case Managers regarding clinical criteria and appropriate use of screening tools.</li>
</ul>
CLINICAL DOCUMENTATION INGERITY and QUALITY REPORTING SUPPORT:
<ul>
<li>For Medical Staff
<ul>
<li>Educates individual hospital staff physicians about ICD-10 and DRG coding guidelines (e.g., co-morbid conditions, outpatient vs. inpatient) and clinical terminology to improve their understanding of severity, acuity, risk of mortality, and DRG assignments on their individual patient records.</li>
</ul>

<li>For CDI and Coding Team
<ul>
<li>Collaborate to develop complaint query practices.</li>
<li>Collaborate to optimize CDI and Coding review process.</li>
<li>Provide necessary clinical support when requested by the team in DRG assignment.</li>

DENIAL MANAGEMENT AND PREVENTION:

<li>Provide necessary education and feedback to providers regarding denials and improve documentation strategies to prevent denials.</li>

PHYSICIAN LIAISON:

<li>Conducts physician education sessions to share data, trends, practice patterns, and other relevant information as requested.</li>
<li>Proactively reports practice pattern trends and opportunities to service line or department specific meetings at the request of the CMO or hospital leadership.</li>
<li>Proactive approach to optimize the service line revenues (professional billing)</li>
</ul>
ORGANIZATIONAL PROCESS AND QUALITY IMPROVEMENT:

<li>Collaborate with Healthcare Data Team in identifying areas or process contributing to excessive cost of care.</li>
<li>Upon request, actively participate in Hospital committees to support to develop protocols related to evidence-based medicine and support optimal standards of care.</li>
<li>Support Payor Contract Process</li>
<li>Support physician contract process for quality measures.</li>
</ul>
OUTCOMES AND DELIVERABLES:
<ul>
<li>Documents education sessions for medical staff on trends, practice patterns, or relevant information. </li>
<li>Develop KPI (dashboard) to monitor progress of revenue integrity</li>
<li>Collaborate with CFO to identify short term and long-term goals.</li>

This position is responsible for the timeliness and accuracy of patient account billing and payer reimbursement by ensuring that coordination of benefits is accurate, and that payer reimbursement is appropriate as agreed upon in our organizational payer contracts. The Billing Representative provides outstanding customer service to our customers and payer sources.

Here's what you will be doing:
• Pursues and participates in education for assigned payer to remain current with changes in the healthcare industry via payer websites, payer newsletters, webinar and attendance at assigned payer meetings.
• Reviews accounts each day in assigned Epic Work Queues and prioritize by dollar amount and timely filing rules to achieve the best possible reimbursement.
• Completes necessary research to submit claims to appropriate payer based on coordination of benefits and active coverage at time service was rendered.
• Evaluates payer remittance advice and/or explanation of benefits on denials for accuracy according to individual payer contracts.
• Maintains knowledge of Medicare, Medicaid, and all commercial insurance programs, including Medicare HMO’s and State Programs to include filing deadlines, billing requirements, and reimbursement methods.
• Composes and submits appeals to third party payers, appealing to the highest level to resolve account denial for proper reimbursement.
• Receives and researches patient, payer, attorney, and third party inquiries received through incoming calls and correspondence in a timely manner by reviewing all available information including payer remits, contracts, documentation, policies and procedures, and insurance laws to formulate an informative response and direct the account appropriately.
• Maintains or exceeds performance metrics according to job assignment focusing on reduction to Accounts Receivable Days and Denial Rates.
• Provides feedback to management on denial trends and claim edits to assist with process improvement and quality assurance within our billing software.
• Provides exemplary customer service and demonstrate positive communication skills in a courteous, accurate and honest manner.
HOME CARE: Performs intake responsibilities and obtains patient insurance benefits information ensuring pre-certifications, authorization and referral requirements are met prior to the delivery of home care services. Maintains current authorizations and manages work queues. Performs other duties as assigned by Home Care leadership.
