Essential duties and responsibilities:

<li>Travel across the 21 counties of the Texas Panhandle to assist and manage a caseload of people who have been recently discharged from psychiatric hospitalization or at-risk of hospitalization</li>
<li>Travel is required within TPC's 21 county catchment area daily to provide treatment services. Our regional clinics are in Borger, Clarendon, Dumas, Hereford, Pampa, &amp; Perryton. </li>
<li>Work alongside other TPC providers in the rural areas to identify clients most in need of intensive services. Some of these clients will be returning to the rural communities after psychiatric hospitalization and are in need of ongoing mental health treatment and support.</li>
<li>Focus on learning rural resources and providing outreach to improve access to care</li>
<li>Visit hospitals to meet with clients, establish care plans, and continue providing care after the clients are discharged and move back into their community</li>
<li>All services will be provided using evidence-based practices that are person-centered and trauma-informed</li>
<li>Need to have the skills necessary to build rapport and establish trusting relationships with clients right after a crisis situation</li>
<li>Introduce clients to a range of services offered by TPC and other community services and will provide care coordination to facilitate access to these resources</li>
<li>Participate in multidisciplinary team meetings and case staffing's to develop treatment approaches that best fit the needs of the client</li>
<li>Maintain same day next day documentation requirements and a 95% or higher compliance average for all individuals served</li>

Minimum qualifications to be successful in job:

Qualifications  &nbsp;
<ul>
<li>Must have and maintain an active, unencumbered license (LPN/LVN) in the state in which the clinician will practice</li>
<ul>
<li>Compact licenses must be transferred to your state of residence within 90 days  &nbsp;</li>
</ul>
<li>Current CPR certification (with hands-on component)- Aveanna can assist in obtaining this requirement after hire, if necessary.</li>
<li>TB skin test (current within last 12 months)&nbsp;&nbsp;&nbsp;</li>
<li>Six months prior hands-on nursing experience preferred but not required&nbsp;&nbsp;&nbsp;</li>
<li>Must have reliable transportation  &nbsp;</li>
