Your primary role is to evaluate and diagnose inmates/detainees/residents (I/D/R) for mental, emotional, and behavioral disorders. In this role, you will:

<li>Perform psychological evaluation on I/D/R population for mental, emotional and behavioral disorders. Provide appropriate diagnosis based on history and presenting symptoms and makes appropriate treatment recommendations.</li>
<li>Evaluate I/D/R’s mental status and severity of symptoms in determining the need for transfer to a higher level of care or placement under observation due to suicide risk or inability to adjust within regular population due to severity of symptoms.</li>
<li>Provide psychotherapy or counseling for the treatment of psychological disorders or mental illness.</li>
<li>Make appropriate referrals as necessary to a psychiatrist or other licensed professional who specializes in prescribing psychotropic medication.</li>
<li>Demonstrate an awareness of the psychotropic medications, monitors medication compliance, and makes referrals to the appropriate licensed medical professional should the I/D/R exhibit possible medication side effects.</li>
<li>Instruct staff and officers in recognizing suicide risk factors and subsequent prevention techniques.</li>
<li>Provide in-service education for medical and correctional staff as required. Performs and trains staff on crisis intervention techniques as required.</li>
<li>Perform administrative duties in keeping accurate records of I/D/R encounters, documenting all services provided, filling out required reports and analyses as required.</li>
<li>Consult with the Physician or Psychiatrist as needed for the proper health care needs of the I/D/R.</li>

General Summary: Assists clients, in their place of residence, with personal healthcare and health maintenance activities as assigned. Includes, but is not limited to, transferring, health maintenance activities, feeding, mobility, and exercises.
 
Responsibilities
<ul>
<li>Supports a patient’s Plan of Care (POC) by monitoring and recording patient condition, providing support and personal services, and teaching families</li>
<li>Monitors and assesses patient condition by observing physical and mental condition, intake and output, and exercise. Performs patient assessments and collaborates with the multidisciplinary care team to develop and implement a plan of care.</li>
<li>Contribute to the continuity of care through effective communication and partnership with the designated Care Team.</li>
<li>Supports patients by providing housekeeping or laundry services, shopping or other household tasks, and preparing and serving meals and snacks</li>
<li>Provides personal services as needed by the client, such as bathing, dressing, and grooming</li>
<li>Helps family members care for their loved one by teaching appropriate ways to lift, turn, and re-position the patient</li>
<li>Interface with family members to field concerns and questions, and educate them on the patient’s treatment and aftercare needs</li>
<li>Records patient information via our EMR system and notifies branch leadership of any changes or unusual conditions</li>
<li>Enhances service reputation by accepting ownership for accomplishing new and different requests and exploring opportunities to add value to our accomplishments</li>
<li>Performs on-call responsibilities and on-call services to patients/families as assigned with a minimum of 10 hours a week which may include nights, weekend and holidays.</li>
<li>Maintains a safe, secure, and healthy environment by following asepsis and universal precaution controls in performing client care</li>
<li>Protects the agency by adhering to professional standards, home care policies and procedures, and federal, state, and local requirements</li>
<li>Maintain mandatory continuing education requirements (CEUs) for the agency/licensing board.d</li>
<li>Must be committed to 30 hours a week as this position is eligible for all company benefits and all financial incentives.</li>

Education &amp; Experience Requirements

<li>High School graduate or equivalent</li>
<li>Home Health Aide Certificate or other similar certification in good standing from an accredited training course that meets the standards established by Health and Human Services</li>
<li>Minimum of one year (1+) of experience as a home health aide or certified nursing assistant (CNA)</li>
<li>Strong English language both written and verbal</li>
<li>Active CPR certification required</li>
<li>Valid driver’s license, reliable transportation and insurance required</li>
