Leasing

Position Overview
The Senior Healthcare Economics Data Analyst – Federally Qualified Health Centers (FQHCs) reports to the Senior Vice President of Accountable Care, CINQCARE, Inc., with accountability for data analytics support for the company’s FQHC partners (with particular attention on CMS’s ACO REACH model) and value-based care initiatives. Integrated Services for Behavioral Health (ISBH) is a community-minded, forward-thinking behavioral health organization helping people along the road to health and well-being. We serve Southeastern and Central Ohio with a comprehensive array of behavioral health and other services – working with local partners to promote healthy people and strong communities. Our services are intended to be collaborative and personalized for the individual.
The Care Coordinator's job responsibilities involve service linkage and care coordination, engaging and working with children, youth, and families with significant behavioral health needs. Care Coordination team members should have a thorough understanding of local communities, be skilled at developing working relationships with community agencies, and identify potential community supports for development to assist families/caregivers working collaboratively with Child and Family Teams. Care Coordination staff ensure children, youth and families have a voice and choice in all coordinated care and services provided.
The pay range for this position is $20.19 - $25.03 per hour based on experience, education, and/or licensure.
Essential Functions:

<li>Joins with family to identify care coordination needs/services in line with service delivery standards and program outcomes to ensure the best outcomes for children, youth, and families.</li>

Minimum Requirements:

<li>Experience providing services and/or support to children and families connected to behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral healthcare field:</li>

<li>two years with an associate degree or bachelor's degree; or</li>
<li>one year with a master's degree or higher</li>
<li>Those with lived experience are encouraged to apply.</li>
<li>Knowledge and experience in Hi-Fidelity Wraparound preferred (Certification provided at time of employment).</li>
<li>Two years of experience in a coordinated supportive services or care coordination role preferred.</li>
