The Nurse Practitioner provides primary care to participants. Performs physical assessments of new PACE Enrollees, semi-annual reassessments of participants and develops and implements appropriate plans of care for program participants. Evaluates participant physical complaints and provides appropriate treatment. Provides participants and caregiver teaching and education. Functions as a member of the Interdisciplinary Team (IDT). Demonstrates the knowledge and skills necessary to assess, plan, care for, and provide services to frail elder participants according to assigned responsibilities and Senior Community Care standards.
QUALIFICATIONS:

<li>Graduate from a school of professional nursing required.</li>
<li>Licensed as Registered Nurse (R.N.) in the state of Colorado.</li>
<li>Certified as a nurse practitioner by a national certifying organization recognized by the State Board of Nursing.</li>
<li>Certified as a Geriatric Nurse Practitioner (G.N.P.) with prescriptive authority preferred.</li>
<li>Minimum of one year’s experience in working with the frail and elderly population required.</li>
<li>Two (2) years' experience as a nurse practitioner in a geriatric setting desirable. Shall have either training or related experience in the job assigned.</li>
<li>Must have medical clearance for communicable diseases and up-to-date immunizations after having direct participant contact.</li>
<li>Must have a valid driver’s license, proof of insurance and have means of transportation.</li>

<li>Participates in the on-call rotation.</li>

<li>Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participants in Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.</li>
<li>Provides relevant participant and caregiver education. Educates and trains participant and caregivers in appropriate care and interventions.</li>
<li>Provides staff in-service on a variety of relevant topics.</li>
<li>Completion and documentation of appropriate diagnostic coding.</li>
<li>Assists with the development of, standards of care; performs on-going monitoring and evaluation of patient care practice and service delivery; provides guidance and training to staff regarding medical and quality assurance issues.</li>
<li>Acts within the scope of his or her authority to practice.</li>
<li>Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants and families.</li>
<li>Follows all Senior CommUnity Care policies and procedures and Occupational Safety and Health Agency (OSHA) safety guidelines.</li>

<li>Provides information about Senior CommUnity Care Program to interested individuals and groups in adherence to PACE regulations.</li>

Job Summary
Working as an Operations Controller&nbsp;you will be responsible for the accounting activity within an assigned account(s) and assisting the Regional Director of Finance with all aspects of the financial activities that occur within the region.

Key Responsibilities:

<li>Oversees all cash handling procedures</li>
<li>Performs reconciliation and analysis for several balance sheet accounts</li>
<li>Analyzes financial results for both internal and external use</li>
<li>Tracks and maintains monthly P/L and balance sheet activity</li>
<li>Develops forecasts and budgets, identifying risks and opportunities</li>
<li>Reviews general ledger and balance sheets</li>
<li>Resolves various accounting/reconciliation issues</li>
<li>Completes financial reports: monthly, quarterly and annual reports, including but not limited to monthly and fiscal close, internal and external audit reporting; manages report preparation and distribution</li>
<li>Maintains inventory and cost control procedures</li>
<li>Performs administration of Payroll and Human Resources</li>
<li>Performs related duties and special projects as assigned</li>
</ul>

Preferred Qualifications:
<ul>
<li>Bachelor’s degree in Accounting or similar field required</li>
<li>Experience managing financials for multi-unit operations is required</li>
<li>Robust experience managing profit/loss (P&amp;L) and fee contracts is required</li>
<li>A minimum of five years’ experience with revenue control, financial reporting and cash handling responsibilities</li>
<li>Accounting experience in foodservice or retail industry required, foodservice operations experience a plus</li>
<li>Ability to prepare monthly financial statements, key metric reports, budgets, forecasts, cash flow projections and analyses</li>
<li>Adept at planning, organizing and controlling complex processes as well as analyzing moderately complex financial information</li>
<li>Experience with POS, cash and credit card reports</li>
<li>Excellent verbal and written communication skills</li>
<li>Skilled at managing multiple priorities and relationships</li>
<li>Computer skills: proficient in Microsoft Office with a concentration in Excel</li>
<li>Strong analytical and organizational skills</li>
<li>Ability to multitask and prioritize in a fast-paced, dynamic work environment</li>

<li>Nationally recognized as a Certified Medical Assistant (CMA) strongly preferred </li>
<li>Strong computer skills required. EMR experience preferred </li>
<li>Some MA Experience required</li>
<li>Current BLS card</li>

Preferred Education/Experience
<ul>
<li>1-3 years of experience in Family or Pediatrics</li>
