Position Summary
We are seeking a skilled and compassionate Registered Nurse (RN)/ Licensed Practical Nurse to provide care in a home healthcare agency. The Field Nurse is responsible for delivering high-quality patient care, conducting assessments, coordinating services, and ensuring compliance with all regulatory standards.
Key Responsibilities
- Perform comprehensive patient assessments, including OASIS documentation - RN only
- Develop, implement, and update individualized care plans - RN only
- Provide skilled nursing care and monitor patient condition
- Coordinate care with physicians, caregivers, and interdisciplinary teams
- Educate patients and families on care plans, treatments, and disease management
- Complete timely and accurate clinical documentation
- Supervise and evaluate home health aides and LPNs
- Participate in case conferences and care coordination activities
- Ensure compliance with Medicare, Medicaid, and regulatory requirements
Qalifications
- Active Registered Nurse (RN) license in the applicable state
- Graduate of an accredited school of nursing
- Minimum 1 year of clinical experience (home health preferred)
- Knowledge of Medicare/Medicaid guidelines and home health regulations
- Strong assessment, documentation, and communication skills
- Current CPR certification
- Valid driver’s license and reliable transportation
- Ability to work independently and manage time effectively
- Willing to travel to clients' homes to provide care
Work Environment & Requirements
- Travel to patient homes required
- Ability to perform physical tasks including standing, walking, and assisting patients
