\nPlans and implements specific treatment programs for individual patients/residents according to the principles and practices of physical therapy.\n
\n
\nPrimary Duties \n
\n
\n1. Evaluates assigned patients/residents and develops treatment plans, including specific goals and activities, \n
\n reevaluating and modifying plans as necessary. Ensures treatments and care is done appropriately and completely \n
\n in accordance with patients/residents clinical condition.\n
\n
\n2. Provides instructions in therapeutic procedures to be continued by the patient as prescribed by a physician. \n
\n Provides direct physical therapy to patients/residents.\n
\n
\n3. Communicates clearly and tactfully with patient/resident regarding progress, problems, home programs and other\n
\n issues related to the therapeutic process.\n
\n
\n4. Communicates observations, facts and comments with interdisciplinary team members individually, and \n
\n through team meetings as appropriate and required.\n
\n
\n5. Assesses patient's progress weekly or as required and adjusts program as appropriate.\n
\n
\n6. Prepares written documentation as requires by regulations and departmental policy, such as evaluations, \n
\n individualized treatment plans, progress notes and other required reports\n
\n
\n7. Provides direct Physical Therapy to patients per plan pf care.\n
\n

\n9. Maintains established departmental policies, procedures, objectives, and quality assurance.\n
\n
\n10. Performs above primary duties for ages adolescent through older adult.\n

\nPhysical demands: \n
\n• May be exposed to infectious and contagious diseases, body fluids and wastes\n
\n• Heavy physical effort, including lift/carry up to 50 lbs\n
\n• Prolonged standing/walking, bending and squatting\n
\n• Must be able to lift, position and transfer patients/residents and equipment\n
\n
\nWorking Conditions:\n
\n• Handles Emergency or Crisis Situations\n
\n
\nMinimum Education/Certification/Experience:\n
\n• Current California Physical Therapy License\n
\n• Current BLS CPR certification\n
\n
