- Must be able to perform all job functions and duties that are required of the Direct Care Staff. Must hold and maintain all certifications throughout employment.
- Ability to supervise staff.
- Participates in developing the Overall Plan of Service and quarterly reports for the client, including developing and implementing individual programs.
- Implement overall plan of service ensuring that all programs are implemented in a timely manner that all staff are in serviced, follow individual programs and document accordingly.
- Monitor medical, physical, and behavioral changes in individuals on a daily basis. Document and report any and all changes to the Director of Operations, Nurse, Behaviorist, as needed.
- Ensure that administration of medication according to policy occurs. Ensure that it is documented properly. Report all errors of medication dispensation or documentation according to policy.
- Ensure that written dietary plans are followed for each client.
- Ensure the cleanliness of the Day Program areas. Ensure the upkeep and maintenance of agency vehicle and request any maintenance needs as they occur.
- Adhere to HPE’s Confidentiality Policy.
- Adhere and enforce all of HPE’s policies and procedures.
- Conduct oneself in a professional manner when interacting with supervisors, co-workers, consultants, families, guardians and members of the community.
- Conduct monthly staff meetings, documenting content and attendance. Provide mandatory training to all staff i.e. resident’s programs, day programs, individual needs, and all aspects of their care.
- Ensure that clients follow a daily schedule, which includes: a variety of community-based outings, programs, leisure activities and positioning guidelines.
- Maintain staff schedule to program ratio, and ensure that staff is in place at all times. Fill staffing absenteeism. Report absenteeism and any staff change to the Director of Operations on a daily basis. Document and track attendance and all forms of communication for all employees.
- Be responsible for the overall running of the Day Program; this includes scheduling activities, outings, van runs, appointments and staffing. The monthly calendar will be reviewed and approved by Director of Operations at least two weeks prior to the start of the new month.
- Report monthly to agency via tickler, re: staff in-services, Fire Drill Report, alarm testing (if appropriate), verification of emergency bags and their content.
- Monitor compliance to safety requirements. Conduct fire drills monthly. Maintain fire extinguishers and other safety related systems.
- Ensures that all necessary paperwork is completed and distributed appropriately, correctly, accurately and in a timely manner, including Incident Reports.
- Be responsible for the accountability of client daily funds, and petty cash including receipt accountability.
- Transport or arrange transport for client as scheduled to recreational outing or to and from day program.
- May be required to work residentially in the event of Day Program Closing. (i.e. snow storm, pandemic, or for any other reason)
- All other duties as assigned.
JOB REQUIREMENTS
- Employee must possess a valid CT driver’s license, a vehicle in well maintained condition, valid registration and auto insurance. Must have a good driving record.
- Must have a working vehicle during a scheduled shift with fuel to transport clients as needed.
- Employee must read, write and speak English. Education: High School Diploma or GED equivalent. CNA or 1 year experience in Human Services field.
- Must have CPR, First Aid Certification, and Medication Administration Certification. Must obtain PMT certification within the time frame set, if applicable. All certifications must remain current throughout employment with H.P.E.; active certifications are a requirement.
EDUCATION
- Must have a High School Diploma or equivalent.
- Minimum one year experience in Human Services field. Including but not limited to nurse’s assistant, adult home companion, doctors office, etc.
- Must be able to obtain or have CPR and First Aid prior to start date. Must have CPR, First Aid Certification, and Medication Administration Certification upon hire. Must obtain PMT certification within the time frame set.
WORKING CONDITIONS AND PHYSICAL DEMANDS

- Ability to stand, sit, walk, squat, bend, reach, stoop, pull, push, ascend and descend stairs and ramps, lift and carry up to 35 lbs. and other daily movements.
- Ability to lift or transfer up to 75 pounds occasionally.
- Ability to work in a stressful environment.
- Ability to operate a calculator, computer, copier/fax machine and telephone effectively.
7am-3pm Monday-Friday
