Start Date: September 7, 2026

Location: Portland, Oregon and surrounding areas
\n
Schedule: Day shift, 8-hour shifts (08:00 - 16:30), Monday through Friday. Typically 1:1 patient visits with up to 6 patients per day, 30 productivity points standard. No weekend requirements or on-call duties.

- Home health setting serving adults and geriatric populations with diagnoses including dementia, Parkinson's, ALS, MS, orthopedic post-op, and post-stroke rehabilitation
- Efficient scheduling with grouped patient visits to minimize drive time; occasional travel up to 60 miles (60-90 minute total drive time) per day
- Free parking and IRS standard mileage reimbursement provided
- Business casual or scrubs allowed; flexible dress code with no specific color requirements
- EPIC Remote Client for Home Health and Rover phone app charting system
- 13-week assignment with guaranteed hours (facility may call off up to one shift per two-week pay period)

Requirements:\n

- Active Oregon Physical Therapist license required
- Minimum 1 year of skilled home health experience required
- OASIS documentation experience required
- BLS/BCLS certification required
- COVID-19 primary vaccination series plus one booster required
- Reliable personal transportation required
- Experience with EPIC charting system preferred
- Two professional references covering 1 year within the last 3 years
