Licensed Vocational Nurse (LVN) - Night Shift

Responsibilities:

Provide direct patient care to residents under the supervision of a registered nurse, including administering medications, wound care, and treatments

Conduct initial and ongoing assessments of patients' physical and emotional needs

Develop and implement individualized care plans to meet the unique needs of each resident

Work collaboratively with the interdisciplinary team to ensure comprehensive care and support

Document patient care and progress in accordance with facility policies and regulations

Participate in quality improvement initiatives to continuously improve patient outcomes and satisfaction
Requirements:

Current LVN licensure in the state of Texas

High school diploma or equivalent required; associate's degree or higher in nursing or a related field preferred

Minimum 1 year of experience as an LVN in a long-term care or rehabilitation setting (n/a)
