Personal Care Assistant - Full Time - Part Time
Under the supervision of the Day Center Coordinator, Lead Personal Care Attendant and Health Care Coordinator (RN), the Personal Care Attendant is responsible for working with indirect supervision from the Director of Activities. Responsible for providing and assisting participants of EveryAge PACE with supportive care. Assists with organization and implementation of activity program. Monitors health status and provides a safe environment for participants. Functions as a member of Interdisciplinary Team.
Essential Duties and Responsibilities:
1. Supervises activities of daily living (bathing, dressing, grooming, hair care, mouth care, eating, hand and foot nail care and toileting) as assigned or needed.
2. Creates and participates in group(s) and one-on-one activities in accordance with daily activity calendar and as requested by the Director of Activities.
3. Monitors and assists participants with preparing meals. Cannot provide assistance with participant’s who require total assistance with feeding.
4. Assists, throughout the day, in maintaining clean and orderly day rooms, kitchen and personal care areas.
5. Accompanies enrollee, who do not require hands on assistance, to medical and other appointments.
6. Performs and or assists with laundry, including clothes for participants.
7. Assists the Drivers with the loading and unloading of participants from vehicles. Travels with Drivers to act as an escort for participants as needed.
8. Demonstrates dependability through consistent compliance with scheduled work hours.
9. Participates in the activities of the Interdisciplinary Team as needed and carries out the plan of care as directed.
10. Notes changes in participant’s physical or emotional health and communicates this information to the appropriate staff person.
11. Maintains participants’ medical record and fulfills EveryAge PACE charting and reporting requirements.
12. Assists the Transportation Department with participants’ takes home supplies.
13. Participates in and support Quality Improvement Initiatives.
14. Maintains safe working environment. Follows all EveryAge PACE program policies and procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.
15. Acts within the scope of his or her authority to practice.
16. Maintains the confidentiality of all company procedures, results, and information about participants, clients or families.
17. Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal Precautions.
18. Participates in continuing education classes and any required staff and training meetings.
19. Performs other duties as required or requested.
Minimal Education:
High school diploma or equivalent
Cardio Pulmonary Resuscitation (CPR) certification preferred.
Minimal Experience:
A minimum of one year of experience in working with the frail or elderly population required.
