About the Opportunity
Medix is partnering with a fast-growing, AI-driven healthcare technology company to hire a Care Coordination Specialist.
This is not a traditional call center or back-office role. You will operate at the intersection of patient engagement, complex clinical workflows, and AI-supported care coordination.

What You'll Do
Drive Patient Engagement

Conduct outreach calls for care gap closure, appointment scheduling, chronic condition follow-ups, and intake completion

Ensure patients complete key steps in their care plans

Provide empathetic, professional support across phone and digital channels
Execute Complex Workflows

Submit, track, and follow up on referrals and prior authorizations

Manage edge cases requiring strong judgment and attention to detail

Navigate payer portals and EHR systems to ensure timely completion
Work Alongside AI

Review AI-generated workflow outputs for quality and accuracy

Identify inconsistencies and edge cases

Provide structured feedback to improve systems and processes
Help Build the Future State

Document new workflow processes as they are launched

Identify inefficiencies and suggest improvements

Operate with a high level of ownership and autonomy
Meet Productivity & Quality Standards

Maintain strong volume and accuracy metrics

Balance patient-facing work with focused, heads-down execution
Who We're Looking For
This role is ideal for someone who:

Has 3+ years of healthcare operations experience (prior auth, patient access, care coordination, MA, healthcare call center, etc.)

Is comfortable shifting between patient calls and complex workflow tasks

Thrives in environments where processes are evolving

Is highly detail-oriented and self-motivated

Enjoys solving problems and building new processes

Is tech-fluent and comfortable learning new systems quickly

Has experience working across EHR systems and payer portals
NYC-based candidates are a plus, but the role is fully remote.

9:00 AM - 6:00 PM EST

Structured onboarding and training
IT Policy: $50 per week refundable equipment deposit required for first 10 weeks. The deposit is taken out from the weekly paycheck (No upfront cost)
