Overview
We are seeking compassionate and dedicated Direct Support Care Professionals to provide high-quality care and support to individuals with developmental disabilities, aging needs, or other health challenges. This role involves working closely with residents in various settings, including group homes, assisted living facilities, and community-based environments. The ideal candidate will be committed to enhancing the quality of life for those they serve through personalized care, patience, and professionalism. Prior experience in home health, long-term care, or hospice is highly valued, along with relevant certifications such as CPR and First Aid. Duties
- Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support
- Provide patient observation and monitor vital signs to ensure safety and well-being
- Administer medications following care plans and HIPAA regulations
- Support residents with meal preparation, cooking, and nutrition needs
- Assist with mobility using equipment such as Hoyer lifts or resident lifts for safe transfers
- Maintain a clean and safe environment through light cleaning and sanitation practices
- Document care activities accurately in accordance with care plans and regulatory standards
- Recognize and respond promptly to changes in residents’ conditions or behaviors, including dementia or Alzheimer's-related needs
- Support residents during activities that promote social engagement and community participation
- Follow all safety protocols related to patient monitoring, medication administration, and emergency procedures such as CPR
Skills
- Valid CPR and First Aid certification required; additional certifications such as HIPAA training are preferred
- Experience working in group homes, assisted living, nursing homes, or home & community care settings is highly desirable
- Strong knowledge of dementia care, Alzheimer's care, memory care, autism support, and hospice care practices
- Ability to assist with complex tasks such as heavy lifting and the use of specialized equipment like Hoyer lifts
- Excellent patient observation skills with the ability to monitor vital signs effectively
- Compassionate caregiver with excellent communication skills and patience when working with individuals with disabilities or developmental challenges
- Knowledge of care planning, medication administration, and documentation procedures
- Ability to work independently as well as part of a team in a fast-paced environment
- Familiarity with social work principles related to long-term care and resident support
