LPN Care Coordinator - Memory Care Unit
Job Summary:
We are seeking a compassionate and skilled LPN Care Coordinator to join our team at Eastern Star Home in Oriskany, NY. As a key clinical leader, you will be responsible for the overall coordination, clinical oversight, and day-to-day management of our 14-bed Memory Care Enhanced Assisted Living Unit. This is a LPN Care Coordinator - Memory Care Unit opportunity to make a meaningful difference in the lives of our residents.
Responsibilities:

Coordinate and manage the care of 14 residents with dementia and other cognitive impairments, ensuring individualized care plans are developed and implemented

Provide clinical oversight and supervision to nursing staff, including assessment, planning, and evaluation of resident care

Collaborate with residents, families, physicians, and other healthcare professionals to ensure continuity of care and resolve any issues that may arise

Develop and maintain relationships with residents, families, and interdisciplinary team members to promote a positive and supportive environment

Identify and report any potential concerns or safety issues to leadership and regulatory agencies as necessary

Participate in quality improvement initiatives to ensure high-quality care and service to our residents
Requirements:

Current NY State LPN license

Minimum 0 years of experience in assisted living or skilled nursing facility setting

Strong understanding of dementia care and memory impairment

Excellent communication and interpersonal skills

Ability to work effectively in a fast-paced, dynamic environment
