We're growing, and we're looking for a Home Health and Hospice Liaison who knows how to open doors, earn trust, and turn relationships into real results for patients and families across the South Hills.

You'll have real support behind you, too. You'll work directly with our VP of Business Development, and our clinical teams deliver the kind of care that makes your job easier. It's a lot simpler to promote something you believe in.
Your Territory
You'll cover the South Hills and neighboring areas within Allegheny County that may include, but are not limited to:
- Mt. Lebanon, Dormont, and Castle Shannon
- Bethel Park, Upper St. Clair, and South Park
- Baldwin, Brentwood, and Whitehall
- Pleasant Hills, Jefferson Hills, and West Mifflin
- Clairton and Elizabeth
- Bridgeville and Southview
If you already live and work in this area, you know the roads, the facilities, and the people. That's a head start.
What You'll Do
- Build and grow referral partnerships with hospitals, skilled nursing facilities, personal care homes, and physician offices across your territory
- Be the voice of Anova by showing referral sources how our home health and hospice teams support their patients
- Own your territory plan and set goals for referrals and admissions, with leadership support every step of the way
- Meet with patients and families to introduce home health or hospice services and help them understand their options
- Keep your work organized by tracking visits and referrals in our CRM
- Mentor newer liaisons as our team continues to grow
What You Bring
- At least two years of healthcare sales or marketing experience (home health, hospice, or palliative care experience is a big plus)
- A natural ability to build trust and keep relationships strong
- A valid driver's license and reliable transportation
- Bachelor's degree in marketing, nursing, or a related field preferred
