What You'll Do\n

- Review and verify medication orders for safety and appropriateness
- Collaborate with physicians and interdisciplinary care teams on medication therapy management
- Participate in clinical initiatives such as antimicrobial stewardship, pharmacokinetic dosing, and anticoagulation monitoring
- Conduct medication reconciliations and provide patient education as needed
- Prepare, dispense, and monitor medications in accordance with professional standards
- Support quality improvement and patient safety initiatives
- Utilize electronic medical record systems to document and manage patient care activities

What We're Looking For\n

- Active Pharmacist license in the state of practice
- At least 1 year of pharmacist experience preferred
- Inpatient or hospital pharmacy experience preferred
- Strong clinical judgment and communication skills
- Experience working within an electronic medical record system preferred
- Commitment to patient safety and high-quality care
