Candidate Pay Rate: $65 - $45.5 per hour

Roles & Responsibilities
- Implement individualized treatment plans established by the supervising Physical Therapist
- Provide evidence-based physical therapy interventions to improve mobility, strength, balance, endurance, and functional performance
- Monitor patient progress and report changes in condition to the supervising Physical Therapist
- Educate patients, families, and caregivers on therapeutic exercises, mobility techniques, and safety precautions
- Collaborate with the rehabilitation team, nursing staff, and other healthcare professionals to support quality patient care
- Maintain accurate and timely documentation in compliance with facility and regulatory requirements
Qualifications
- Active Physical Therapist Assistant (PTA) License
- Associate Degree from an accredited Physical Therapist Assistant program
- Current CPR/BLS Certification (if required by facility)
- SNF experience preferred but not required
- Strong clinical, communication, and interpersonal skills
Education
- Associate Degree from an accredited Physical Therapist Assistant program
Knowledge & Skills
- Physical Therapy knowledge
- Patient care and rehabilitation skills
- Excellent communication and interpersonal abilities
- Clinical skills in a skilled nursing setting
