Overview
The Medical Records Clerk maintains and organizes health records to support accurate and timely documentation for CalAIM-related services. The role manages medical records within the EHR, including documentation related to Medi-Cal eligibility screenings, discharge planning, and in-reach service encounters. The position ensures confidentiality, supports care coordination, and assists with audits, reporting, and secure transmission of required health information.
Responsibilities
- Maintain, organize, and ensure confidentiality of medical records, including processing releases of information.
- Pull, file, and manage medical records and related documentation generated inside and outside the institution.
- Perform clerical and technical duties such as preparing reports, handling communications, and scheduling individuals for medical appointments.
- Support quality improvement activities, audits, and preparation of statistical and management reports.
- Ensure secure transmission of discharge care plans, track consent forms, and support compliance with CalAIM documentation standards.
Qualifications
Education
- High school diploma or G.E.D.
Experience
- Training in an accredited medical record program or minimum of one year experience in an established medical record system preferred, but not mandatory
- Prior experience in a medical records or correctional health environment is essential, along with a working knowledge of HIPAA and CalAIM documentation protocols.
