As a Hospice RN Case Manager, you will:

-Provide skilled nursing assessment and care in patients’ homes and communities

-Develop and implement individualized plans of care

-Assess patient and family needs and coordinate appropriate services

-Collaborate with the interdisciplinary group (IDG)

-Communicate effectively with patients, families, physicians, and team members

-Monitor medications, supplies, and equipment for appropriate utilization

-Document care accurately and maintain compliance with hospice regulations and standards

-Assist with On call requirements

Job Requires:

-Current RN license in the State of Ohio

-Hospice, home health, or community nursing experience preferred

-Compassionate, organized, proactive, flexible, and dependable

-Strong clinical judgment and communication skills

-Ability to work independently while being a collaborative team member

-A genuine desire to provide exceptional end-of-life care

Experience:

• NURSING : 1 year (Required)

License/Certification:

• CPR Certification (Required)

• RN License (Required)

Ability to Commute:

• Beachwood, OH and surrounding communities willingness to travel:
