Shift: Day Shift, 8:00 AM – 4:30 PM

Schedule: 5 × 8-hour shifts

Total Weekly Pay: $2,449.00

Job Overview

We are seeking an experienced Master of Social Work (MSW) professional to support the Case Management department in an acute-care hospital setting. The ideal candidate will have acute-care experience and the ability to coordinate patient care, support discharge planning, and collaborate with multidisciplinary healthcare teams.

Key Responsibilities

• Support case management activities in an acute-care hospital environment.

• Assess patient needs and assist with care coordination and discharge planning.

• Collaborate with physicians, nurses, social workers, patients, families, and other healthcare professionals.

• Help facilitate safe patient transitions and continuity of care.

• Manage an approximate caseload of 20–23 patients.

• Maintain accurate documentation and follow established hospital policies and procedures.

• Float to the Laguna facility as required.

Required Qualifications

• Master’s degree in Social Work (MSW).

• Minimum of 1 full year of acute-care experience.

• Previous travel assignment experience required; first-time travelers will not be accepted.

• California state license is not required.
