Psychiatric Mental Health Nurse Practitioner (PMHNP)

This position may work out of Connect Health + Wellness Bassett or Ridgeway clinics but will support patients at each of our Primary Care sites.

Position Summary: The Psychiatric Mental Health Nurse Practitioner (PMHNP) delivers compassionate, patient-centered psychiatric care across the lifespan. This role involves collaboration within a multidisciplinary care team to assess, diagnose, and treat mental health conditions, create individualized treatment plans, and support patients in managing their mental wellness.

Key Responsibilities

Patient Care

  • Perform comprehensive psychiatric assessments, including history, mental status exams, and diagnostic evaluations.
  • Develop, implement, and revise individualized treatment plans.
  • Prescribe and manage psychiatric medications in accordance with clinical guidelines and applicable regulations.
  • Provide psychotherapy (individual, group, or family), as appropriate.
  • Educate patients and families on diagnoses, treatment options, and preventive strategies.

Interdisciplinary Collaboration

  • Collaborate with primary care providers, social workers, and other team members to deliver integrated care.
  • Actively participate in team meetings to review and adjust patient care plans.
  • Serve as a resource for staff regarding behavioral health concerns.

Documentation

  • Maintain accurate, thorough, and timely documentation in the electronic health record (EHR) system in accordance with organizational and regulatory standards.

Quality Improvement

  • Engage in continuous quality improvement efforts to enhance clinical outcomes and service delivery.
  • Stay informed on current best practices and evolving trends in psychiatric care.

Community Engagement

  • Support mental health education and outreach efforts to reduce stigma and promote awareness.
  • Collaborate with community organizations to improve access to behavioral health resources.

Other Duties

  • Perform additional duties as assigned by the supervisor that are aligned with the mission and scope of this role.

Pay Range: $96,000 to $120,000
