Requirements

Active, unrestricted Louisiana medical license

Board Certification in hand; BC/BE in EM, FM, or IM preferred

Active Louisiana DEA with a Louisiana address

Louisiana State Controlled Substance License

Supervising Physician License through the Louisiana State Board of Medical Examiners
-
Completion of an approved 3-year EM, FM, or IM residency
-
ACGME/AOA-approved residency training
Certifications
If not Emergency Medicine trained:

ACLS
-
ATLS
-
PALS
Clinical Requirements
-
Experience/privileges for Moderate Sedation and Deep Sedation

If not EM board certified, completion of the SCP University Procedural Sedation Version 2 course is required
Credentialing Requirements
-
Flu vaccination within the current flu season
-
TB testing within the past 12 months or chest X-ray with positive TB history
-
Case logs from the previous 2 years
-
CME documentation from the previous 2 years
Malpractice
-
$100K/$300K malpractice coverage
-
Enrollment in LPCF
-
Locums providers must maintain LPCF enrollment for a minimum of 90 days at a time
Resident Information
3rd-year residents are accepted for moonlighting privileges only through June 30. Upon completion of residency, a new application through CVO will be required.
Important: Candidates must have the required state license, board certification, and active DEA/CSR in hand to be presented.
