Job Description: RN Case Manager
Position Summary:
The RN Case Manager is responsible for coordinating and providing skilled nursing care to hospice patients in accordance with the individualized plan of care, physician orders, hospice policies, and applicable federal and state regulations. Responsibilities include conducting patient assessments, managing symptoms, coordinating care with the interdisciplinary group (IDG), communicating with physicians and families, completing timely clinical documentation, and ensuring continuity of care.
Essential Duties and Responsibilities:
- Assess patient conditions and identify changes in health status.
- Develop, implement, and update individualized nursing care plans.
- Provide skilled nursing care and symptom management.
- Coordinate patient care with physicians, CNAs, social workers, spiritual care staff, and other IDG members.
- Participate in IDG meetings and patient care planning.
- Educate patients and caregivers regarding disease processes, medications, and comfort measures.
- Maintain accurate, timely, and complete clinical documentation.
- Ensure compliance with CMS hospice Conditions of Participation, Georgia regulations, and agency policies.
Educational and Licensing Requirements:
- Graduate of an accredited school of nursing with an Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN).
- Current, unrestricted Registered Nurse (RN) license in the State of Georgia.
- Current CPR certification, as required by agency policy.
- Previous hospice, palliative care, or home health experience preferred.
- Knowledge of hospice philosophy, symptom management, care planning, and applicable regulatory requirements preferred.
