Elite Home Healthcare, LLC is expanding our nursing team serving Bedford, Indiana, and surrounding Lawrence County communities. We're looking for experienced Home Health Registered Nurses (RNs) from Bloomington, Bedford, and surrounding South Central Indiana communities interested in an RN Case Manager opportunity involving patient assessments, OASIS documentation, individualized care planning, and coordination of Medicaid home health services.

As an RN Case Manager with Elite, you'll help patients manage chronic conditions, maintain or improve their health, and receive individualized care in their homes. You'll combine your clinical nursing experience with care coordination, patient education, documentation, and collaboration with physicians and our interdisciplinary team.
We are particularly interested in Registered Nurses with home health experience, OASIS experience, strong clinical assessment skills, and excellent documentation and time-management skills.
Work territory: This is a field-based position serving patients in Bedford and surrounding Lawrence County communities. Bloomington-area nurses are encouraged to apply; travel to assigned patient homes within the Bedford territory is required.

What You'll Do
As an RN Case Manager, you'll serve as a primary clinical resource for assigned home health patients, helping coordinate services, monitor patient progress, and support continuity of care.
Responsibilities include:
- Completing comprehensive Start of Care (SOC) assessments and OASIS documentation, when applicable
- Performing required reassessments, recertifications, transfer assessments, resumptions of care, and discharge assessments
- Developing and initiating individualized Plans of Care that establish patient-centered goals
- Managing and coordinating care for Medicaid home health patients
- Assessing patient needs and monitoring changes in condition
- Helping identify and address changes that could contribute to deterioration or unnecessary complications
- Supporting patients in managing chronic conditions and maintaining or improving their health
- Completing accurate and timely admissions, recertifications, discharges, and related home health documentation
- Communicating changes in patient condition and care needs to the appropriate physician or ordering practitioner and members of the care team
- Collaborating with the interdisciplinary team to coordinate patient services and support continuity of care
- Providing verbal reports and participating in staff meetings and care conferences as requested
- Maintaining documentation in accordance with applicable requirements and Elite Home Healthcare policies
RN Case Manager QualificationsRequired
- Current, active Registered Nurse (RN) license authorizing practice in Indiana
- At least 2 years of nursing experience
- At least 1 year of home health nursing experience
- Knowledge of home health assessments, clinical documentation, and patient-centered care planning
- Excellent written and verbal communication skills
- Strong organizational and time-management skills
- Proficiency with Microsoft Office and the ability to work with electronic clinical documentation systems
- Ability to travel to assigned patient homes throughout the Bedford/Lawrence County territory
Preferred
- Previous RN Case Manager experience
- Experience completing OASIS assessments
- Experience with Start of Care, recertification, transfer, resumption of care, and discharge documentation
- Experience coordinating care for Medicaid home health patients
- Experience managing patients with chronic or complex medical conditions
